Anamnese
Tijdens de anamnese moet gevraagd worden naar de duur en het beloop van eventuele klachten, risicogroepen en risicocontact moet worden uitgevraagd, een doorgemaakte hepatitis in de voorgeschiedenis en vaccinatie tegen hepatitis A of B moet worden uitgevraagd. Verder is het gebruik van alcohol of drugs van belang bij verdenking op hepatitis.
Risicogroepen voor virale hepatitis
De risicogroepen bestaan uit (gezins)contacten met besmette personen, kinderen in opvangcentra, reizigers, personen afkomstig vanuit endemische gebieden, een positieve familieanamnese of verhoogd beroepsrisico, mensen met wisselende seksuele contacten, drugsgebruikers en verstandelijk gehandicapten. De risicocontacten voor virale hepatitis zijn besmet voedsel, drinkwater, ontlasting en urine, ejaculaat en vaginaal vocht, bloed, prik-, snij-, spat- en bijtincidenten, een moeder met dragerschap en een niet-steriele ingreep.
Lichamelijk onderzoek
Het lichamelijk onderzoek bij verdenking op virale hepatitis bestaat uit de inspectie van de sclerae, percussie en palpatie van de lever en galblaas. Wanneer een vermoeden voor levercirrose bestaat, moet gelet worden op de aanwezigheid van spider naevi, erythema palmare, gynaecomastie, flapping tremor, testisatrofie, ascites, splenomegalie, veneuze collateralen in de buikwand.
Aanvullend onderzoek
Het aanvullend onderzoek bij verdenking op virushepatitis bestaat uit IgM-anti-HAV bij hepatitis A, HBsAg bij verdenking op hepatitis B, anti-HCV bij verdenking op hepatitis C. Wanneer de ALAT/ASAT verhoogd is, moet de bepaling nog eens gedaan worden na ee maand.
Beleid bij virale hepatitis
Het beleid bij een virale hepatitis bestaat uit voorlichting geven over de besmettelijkheid. Wanneer iemand hepatitis A heeft moet diegene thuis blijven en er moet actieve immunisatie plaatsvinden bij de omgeving. Wanneer iemand hepatitis B heeft moet diegene geen onveilig seksueel contact meer hebben. Wanneer iemand hepatitis C heeft moet contact met het bloed van diegene vermeden worden.
