Werkcollege 1: Diagnostiek bij niet-westerse migranten - Neuropsychologische Diagnostiek
Is migratie een risico? Migranten hebben gemiddeld minder jaren onderwijs, wat betekent dat er minder cognitieve reserves zijn. Ook is er sprake van een lagere socio-economische status, wat je levensstijl beïnvloedt. Roken bijvoorbeeld is ook een risico. Etniciteit zelf is geen risicofactor voor het krijgen van cognitieve aandoeningen; sekse ook niet.
Hoe neem je tests af als de moedertaal niet Nederlands is?
- Bestaande testen vereenvoudigen
- Een meer kwalitatieve dan kwantitatieve beoordeling
- Kijken naar het gehele profiel
- Tolk
Diagnostiek is lastig want:
- Taalbarrière
- Schoolse vaardigheden (jaren onderwijs)
- Inhoud niet herkenbaar of begrijpelijk
- Validiteit en betrouwbaarheid in geding
Je kunt dus wel de test gaan vertalen, maar nog steeds is de validiteit en betrouwbaarheid in het geding, omdat het niveau vaak nog te hoog ligt.
Mensen die niet-geletterd zijn blijken moeite te hebben met de visuele associatie test. De reden lijkt te zijn dat het lastig is om een plaatje in woorden om te zetten bij mensen met weinig jaren onderwijs. Realistische plaatjes (ipv tekeningen) gebruiken helpt al een stuk.
Welke geheugentesten zijn bruikbaar?:
- WMS
- Complex figuur van Rey
- Trail Making Test
- Kloktekening
Er is een Cross-Culturele Dementiescreening (CCD), die geschikter is omdat:
- Geen gesproken respons of uitleg
- Geen items die beroep doen op schoolse vaardigheden
- Inhoud moet herkenbaar zijn voor personen uit allerlei culturen
- Uitleg over verwachtingen tijdens test: verwachtingen uitleggen.
Corsi Block-Tapping Test is een alternatief voor de WAIS-cijferreeksen test.
Beperkingen CCD:
- Geen interbeoordelaarsbetrouwbaarheid
- Screening omvat niet alle domeinen
- Multitasken tijdens afname
- Extra instructie van omgeving
- Of: moeilijk om te weten of patiënt het niet begrijpt of het niet kan
Een alternatief voor de MMSE is de RUDAS. Hierbij wordt het volgende getest:
- Geheugen: boodschappen onthouden
- Woordvloeiendheid: dieren
- Praxis: gebaren nadoen
- Visuoconstructie/EF: kubus tekenen
- Oordeelsvermogen: oversteken
Redenen voor niet (of laat) inschakelen van specialistische hulp kunnen zijn schaamte, dementie niet zien als ziekte of als onderdeel van normale veroudering, schaamte en stigma
Dus: wat maakt diagnostiek bij niet-westerse migranten zo moeilijk? De taal, culturele verschillen, begrijpelijkheid van de inhoud, laag geletterd.
