Hoe is gedrag te diagnosticeren?
Twee algemene belangrijke termen in dit artikel zijn Diagnostiek op micro- en macroniveau. Bij diagnostiek op microanalyse niveau ligt de focus op de wisselwerking van cognitieve, affectieve en gedragscomponenten. Op macroanalyse niveau wordt nagegaan hoe problematiek in stand wordt gehouden door de ruimere context.
Microniveau: het G-schema
Gedrag wordt opgedeeld in ‘gedachten’ (cognitief), ‘gevoelens’ (affectief ) en ‘gedrag’ (motorisch). Een vierde componend, ‘gewaarwordingen’, kan ook toegevoegd worden. Ze beïnvloeden elkaar voortdurend in positieve en negatieve zin. Externe gebeurtenissen zijn ook van invloed en brengen het systeem van gedachten, gevoelens, gedrag en gewaarwordingen in beweging. Uiteindelijk vloeien er ‘gevolgen’ uit voort. Bij ‘gevolgen’ wordt onderscheid gemaakt tussen interne en externe gevolgen. Interne gevolgen zijn als een organisme functioneert in een positieve of negatieve beweging of spiraal van denken, voelen en handelen, heeft dit invloed op de kwaliteit van elk afzonderlijk component. Als iemand met een negatief zelfwaardegevoel steeds moeilijke situaties uit de weg gaat, kunnen de negatieve zelfgerelateerde gedachten toenemen, omdat het verarmd functioneren en een tekort aan positieve ervaringen de negatieve opvattingen bevestigen. Negatieve cognities over zichzelf breiden zich uit.
Externe gevolgen gaat over extern waarneembaar gedrag dat effect heeft op de omgeving en leidt tot externe gevolgen. Door te handelen verandert de materiële omgeving en wordt de sociale omgeving beïnvloed. Deze omgeving zal het extern waarneembare gedrag interpreteren en vanuit die interpretaties reageren. Gedragstendensen zijn processen die gepaard gaan met de toenemende emotie en die het organisme voorbereiden op het verrichten van een handeling. Bijv. spanning en gejaagdheid. Als felle emoties aanwezig zijn, zullen actietendensen vaak ook leiden tot handelingen zoals agressie of wegvluchten. Belangrijk bij een dwanghandeling is dat de patiënt het onderscheid leert ervaren tussen actietendens die wordt geactiveerd (drang om te controleren of vermijden) en de keuzemogelijkheid om er al dan niet aan toe te geven (het uitvoeren van het dwanggedrag).
Behandelingen met het G-schema
Exposure therapie activeert én gedragstendens (drang) én vergroot de keuzemogelijkheid van de patiënt om er geen gevolg aan te geven en alternatief gedrag te vertonen. Functionele analyse maakt een onderscheid tussen het klassieke en operante component. Het klassieke component, ook wel betekenisanalyse, behandelt de vraag waarom een stimulus bij deze persoon een bepaald gevoel veroorzaakt. Er wordt nagegaan welke besefte of niet besefte betekenissen worden geactiveerd naar aanleiding van de confrontatie met een stimulus. Bij emotionele klachten zullen we nagaan welke betekenissen worden geactiveerd, die dan leiden tot disproportionele of storende emoties.
Macroniveau: de invloed van omgeving en persoonlijkheid
Bij het analyseren van gedrag is het belangrijk om rekening te houden met de context waarin het plaatsvindt. De methode die binnen de gedragstherapie gebruikt wordt is het opstellen van een holistische theorie (ook wel probleemsamenhang genoemd). Hierin wordt in kaart gebracht hoe levensomstandigheden, persoonlijkheidskenmerken en copingstijlen met elkaar interacteren en problematisch gedrag kunnen verklaren. De holistische theorie is hypothetisch, en dus moet deze getoetst worden. Dit wordt gedaan door de theorie te bespreken met de cliënt, observatieopdrachten en vragenlijsten. Wanneer de holistische theorie bevestigd wordt, kan deze gebruikt worden als leidraad voor behandeling. Wel moet bedacht worden dat de theorie na verloop van tijd niet meer geldig is, vanwege veranderingen in de tijd. Bij het analyseren van gedrag is het belangrijk zowel op micro- als macroniveau te denken. De diagnostiek wordt hierdoor gedetailleerder en er komt dus beter begrip van de problematiek uit voort. Wel zijn beide methoden, functie-analyse voor microniveau en holistische theorie voor macroniveau, beperkt geldig.
