“In the context of digital mental health interventions”

Therapeutic alliance: key role affects therapy -->

Can we do the same in an online interaction?

  • Clients can feel a strong alliance with what the digital mental health intervention is offering --> but not same effects, it supports other aspects

    • (Online settings) Client feels presence therapist, boosts the therapy effect

      • Increased engagement
      • Increased adherence
    • (Online settings) Enhance effects in a bidirectional way: therapist and client affect each other in a complex way, when there is 3rd system (online material) emerging

Therapeutic alliance by Bordin (1979):

  1. Bond client/therapist
  2. Agreement on tasks
  3. Therapeutic goals
  • Digital mental health interventions (DMHI): (1) rated low, (2) and (3) rated the same

Persuasive Systems Design (PSD):

To design a DMHI in such a way that it’s supports what the therapist is already doing,

  • Use aspects from DMHI in specific parts of therapy (e.g. use reminders --> it supports the task of the therapist)

    • Tailoring & Personalization: e.g. tailoring to a person’s stage of recovery
    • Task support: e.g. positive feedback, reminders,…
    • --> Enhances self-efficacy: locus control of the client is more present with DMHI

Therapeutic feedback factors:

  • Working alliance (complimenting, reflecting worries)

    • More related to outcomes in physical context
  • Therapy integrity (goal orienting, encourage homework)
  • Positive attitude (understanding, positive framing)
  • Sleep expertise (sleep efficiency, bedtime advice)
    • More related to outcomes in online context
