Hoorcollege 7: Stemmingsstoornissen (Psychopathologie 2, UU)
Er is een enorm spectrum aan stemmingen: van depressie tot euthymie tot manie.
De diagnose bipolaire stoornis is erg lastig, omdat het zich verspreid over een langere tijd.
Medicamenteus is lithium de 1e keus. Daarnaast werkt psychoptherapie ook; herkennen van de triggers en de invloed van de stoornis. Richt zich ook op zelf management en bio/sociaal ritme.
Er is weinig bekend over de verklaring. Er is een aangeboren biologische disbalans. Genetica speelt ook een rol. Triggers: psychosociale stress: relatie uit/ ontslag / overlijden.
Behandeling depressie: stepped care
- Licht < 3 mnd: psycho-educatie + gedragsactivatie
- Matig ernstig: psychotherapie of psychofarmaca SSRI/SNRI
- Ernstig / recidiverend: combinatietherapie
- ‘Treatment resistant’: ECT / MAO-remmers
Verklaring depressie: 4 richtingen die depressie verklaren
- Gedragsmodel
- Cognitieve model
- Psychodynamisch model
- Systemisch model
Dit zijn de holistische modellen. Echter is het ook belangrijk om naar de neurobiologie te kijken: het neurobiologische model.
- Gedragsmodel: gebrek aan bekrachtiging --> vicieuze cirkel
- Cognitieve model: cognitieve biases op gebied van aandacht, verwerking en geheugen
- Neurobiologisch model: genen leiden via eiwitten tot neurotransmitters, wat hersenactiviteit beïnvloed etc.
Al deze modellen geven samen een goede verklaring voor depressie.
Interpersoonlijke therapie: net zo effectief als CGT. Richt zich op sociale aspecten van de mens. Vier mogelijke focussen:
- Complexe rouw
- Interpersoonlijk conflict
- Rolverandering
- Interpersoonlijk tekort
Treedt veel comorbiditeit op bij depressie.
Comorbiditeit met hart en vaat ziekten, HIV, dementie.
Depressie bij somatische stoornis verslechtert prognose.
Depressie verhoogd kans op dementie.
Eerste depressieve episode op oudere leeftijd (55+) = prodroom van neurodegeneratief syndroom.
Extra opletten therapie ouderen:
- Veranderend levensperspectief
- Existentiële verschillen: groot aantal verliezen: fysiek, sociaal en interpersoonlijk
- Andere relatie met de psychotherapeut
- Apathie alleen is grote risico factor voor dementie!
Take home message:
- Bipolair is meer dan ene stemmingsstoornis
- Stemmingsstoornissen hebben vaak een chronisch beloop
- Behandelingen stemmingsstoornissen zijn matig effectief
- Depressie bij somatische stoornis verslechtert prognose
- Niet alle symptomen van depressie zijn altijd te verklaren vanuit gedrag en/of cognities
- Depressieve ouderen noemen zichzelf zelden depressief
