Anamnese bij ulcus cruris venosum

De anamnese bij verdenking op een ulcus cruris venosum bestaat uit het uitvragen van het ontstaan, de duur en het beloop van de klachten, pijnklachten, een infectie, de kwaliteit van leven en de voorgeschiedenis. Verder moet gevraagd worden naar risicofactoren, zoals diabetes mellitus, oedeem, hartfalen, hypertensie, immobiliteit, varices, reuma, langdurig staan en huidaandoeningen.

Lichamelijk onderzoek

De ulcus moet geïnspecteerd worden en er moet gelet worden op de lokalisatie, grootte, het aspect van de wondrand en de wondbodem. Verder moet gelet worden op tekenen van infectie en aanwijzingen voor onderliggende oorzaken.

Aanvullend onderzoek

Het aanvullend onderzoek bij verdenking op een ulcus cruris venosum bestaat uit laboratoriumonderzoek, waarbij de BNP, Hb, Ht, TSH en glucose bepaald worden. Verder wordt de enkel-armindex gemeten als de a. dorsalis pedis niet palpabel is. Verder kan nog duplexonderzoek plaatsvinden.

Verschil tussen een veneuze of arteriële ulcus

Een veneuze ulcus zit boven de mediale malleolus, heeft grillige wondranden, heeft een onwelriekende geur, pitting-oedeem, een enkel-armindex van groter dan 0,9, nachtelijke pijn en kramp. Verder kunnen varices aanwezig zijn, hyperpigmentatie, een zwaar vermoeid gevoel bij stilstaan en jeuk. Een arteriële ulcus zit aan de laterale zijde van het scheenbeen, op de voorvoet of de tenen, heeft scherpe wondranden, een zwarte wondbodem, geen oedeem, een enkel-armindex lager dan 0,9 en wordt gekenmerkt door veel pijn, nachtelijke pijn, vermindert bij afhangen van het been.

Beleid

Het beleid bij een ulcus cruris venosum bestaat uit voorlichting over de ontstaanswijze van de ulcus, dat het lang duurt tot genezing, dat oedeem bestreden moet worden door zwachtelen en bewegen, dat therapeutisch elastische kousen gebruikt moeten worden en dat er een optie bestaat voor variceschirurgie. De niet-medicamenteuze behandeling bestaat uit wondbehandeling. Deze bestaat uit het reinigen van de wond, debridement van necrose, bescherming wondrand met crème, bedekken van de wond, een secundair verband aanleggen en de ambulante compressietherapie starten. De medicamenteuze behandeling bestaat uit systemische antibiotica bij een uitbreidende infectie, pijnbestrijding en lidocaïneprilocaïnecrème op de wond.

Follow-up

De follow-up bestaat uit drie weken na de start van de behandeling, twee maanden na de start van de behandeling en op indicatie. De wond moet beoordeelt worden, de therapietrouw uitgevraagd worden en er moet gevraagd worden naar de pijn en kwaliteit van leven.
