Zorgpaden
Zorgpad is een ander woord voor zorgprogrammering. Een zorgpad/klinisch pad is een complexe interventie om de gemeenschappelijke besluitvorming en organisatie van zorgprocessen te verwezenlijken voor een specifieke groep patiënten gedurende een gedefinieerd tijdskader.
Een zorgpad is:
- Aandoeningsgericht/klachtgericht
- Beschrijft het pad dat de patiënt doorloopt
- Is evidence based en gestandaardiseerd
- Is expliciet (taak en tijd)
- Beschrijft wat er van de patiënt wordt verwacht (empowerment)
Zorgnetwerk
Een zorgnetwerk is een gecoördineerde manier van samenwerken tussen een groep zorgaanbieders om de gezondheidsuitkomsten van een (deel)populatie te verbeteren. De zorgaanbieders zijn daarbij bereid gezamenlijk risico te dragen voor het wel of niet realiseren van kwaliteit van de zorg.
