Inleiding
De toegankelijkheid van de jeugd-ggz is niet gelijk voor verschillende etnische groepen. In de populatie van de jeugd-ggz worden jongeren van etnische minderheden ondervertegenwoordigd. De situatie in forensische context is hieraan tegengesteld, waarbij jongeren van etnische minderheden oververtegenwoordigd zijn.
Een mogelijke verklaring wordt gegeven in het filtermodel voor de toegang van de ggz. Volgens dit model moeten diverse filters worden gepasseerd voordat behandeling wordt geïnitieerd. Ieder individu moet deze filters passeren, maar voor sommige groepen is dit moeilijker dan voor andere. Dit wordt ‘selectieve filtering’ genoemd.
De populatie jeugddelinquenten wordt gekenmerkt door een hoog percentage psychopathologie. De oververtegenwoordiging van niet-westerse jongeren zou erop kunnen wijzen dat deze populatie pas in een laat stadium van hun stoornis in contact komt met de ggz. Het is belangrijk om inzicht te krijgen in de etnische samenstelling van de jeugd-ggz. Aan de hand daarvan kan de toegankelijkheid verbeterd worden.
Methode
In het onderzoek dat in dit artikel beschreven wordt is bij De Jutters, een jeugd-ggz-instelling in Den Haag, gedurende twee jaar de herkomst van hoofdcliënten en hun ouders geregistreerd. Er is een indeling gemaakt naar herkomstland; herkomst uit westerse en niet-westerse landen; en eerste en tweede generatie-jongeren. Etnische groepen zijn geïncludeerd als zij meer dan 1% van de totale bevolking besloegen. Er is een vergelijking gemaakt tussen forensische en overige afdelingen. Daarnaast is een vergelijking gemaakt tussen de leeftijdsgroepen 0-14 en 15-19, wat de resultaten enigszins vertekend kan hebben omdat 0,5% ouder was dan 19.
Resultaten & conclusie
In de reguliere ggz zijn zowel westerse als niet-westerse immigranten van zowel de eerste als tweede generatie ondervertegenwoordigd in de groep van 0-14 jaar. Deze groepen hebben ongeveer 50% kans op behandeling vergeleken met Nederlandse leeftijdgenoten. In de groep van 15-19 jaar is de ondervertegenwoordiging minder groot, maar deze bestaat nog steeds. In de forensische ggz zijn westerse immigrantengroepen ondervertegenwoordigd, terwijl niet-westerse immigrantengroepen oververtegenwoordigd zijn. Opvallend is dat de Surinaamse en Antilliaans/Arubaanse groep bijna evenveel vertegenwoordigd is als de Nederlandse groep.
Uit de cijfers van het huidige onderzoek kan geconcludeerd worden dat veel immigrantengroepen slecht bereikt worden als zich psychiatrische problematiek voordoet tijdens de vroege jeugd. Om de minderheidsgroepen beter te bereiken, is ‘protoprofessionalisering’ van belang. Dit houdt in dat mensen op de hoogte zijn van mogelijkheden om hulp te zoeken in de zorg. Daarnaast is het noodzakelijk dat er meer inzicht in de etnische bias komt vanuit de jeugd-ggz. Hiervoor is meer onderzoek naar selectieve filtering nodig.
