Indicatiestelling: een beschouwing
Bij een integratief opgevatte intake gaat men uit van het perspectief van de patiënt. De intaker vraagt dit grondig uit zonder onmiddellijk expliciet een vertaling naar een bepaald referentiekader te maken. De diagnosticus luistert en exploreert, vraagt kritisch. Hierbij is het aantal vroege drop outs minder: naarmate de patient de verwijzing voor een bepaalde vorm van behandeling meer als een overleg beoordeelt, is de kans groter dat de patient positieve verwachtingen heeft van de behandeling. Belangrijk om te weten voor de diagnosticus zijn precipiterende factoren, wat ging er vooraf aan de huidige klachten, en predisponerende factoren. Deze omvat de biologische of psychologische kwetsbaarheid.
Het gesprek
Tijdens het eerste gesprek wordt een voorlopige diagnose gesteld, info gegeven over de procedure tijdens en na de intake, toestemming gevraagd om info op te vragen bij andere behandelaars en de patient gemotiveerd voor een bepaalde behandeling. Er zijn verschillende zogenaamde motivatie fases van de patiënt. De bezoeker wil niets veranderen en hierbij heeft behandeling geen zin. De klager wil slechts zijn ei kwijt, hierbij zijn een paar gesprekken genoeg. Tot slot is er de consument, die werkelijk iets wil veranderen.
De geschiedenis van de DSM
DSM-I was sterk theoretisch en weinig empirisch gestuurd. Pas vanaf DSM-III ontstond er een klassificatiesysteem, werd atheoretisch en de vijf assen ontstonden. Nadelen van een DSM klassificatiesysteem is dat er overlap van symptomen kan zijn, en een lage betrouwbaarheid. Vooral bij persoonlijkheidsstoornissen en schizofrenie. Daarnaast is er veel comorbiditeit. Er zijn verschillende logische conclusies over hoe verandering teweeg kan worden gebracht bij een patiënt met een psychische stoornis. Dit kan binnen verschillende referentiekaders:
psychodynamische kader: letten op kenmerken van het ego, ego sterkte, veerkracht, realiteitstoetsing, impulscontrole, oordeelsvorming, vermogen tot psychologisch denken, afweerfuncties, superego, objectrelaties en rijpheid daarvan, de zelf, zelfwaardering en grenzen van het zelf t.o.v. anderen.
cognitief-gedragstherapeutische kader: letten op de automatische gedachten, instrumentele cognities zoals leefregels en basale disfunctionele overtuigingen (schema’s)
systeemkader: onderlinge relaties, interacties en contexten beschrijven. Op basis hiervan een werkhypothese opstellen waarmee problemen kunnen worden aangepakt.
Doelen
De diagnostiek heeft verschillende doelen:

identificeren of iemand risico loopt om een bepaald probleem te ontwikkelen
prognose bepalen
beste behandeling bepalen
tussentijds bepalen van het behandelverloop
evalueren van het effect van behandeling
