Wat houdt de diagnostische fase in?
De diagnostische fase heeft tot doel om een beeld te krijgen van de pathologie waar een patiënt mee te maken heeft. Hierin komen de volgende onderwerpen aan bod: huidige klachten, ziektegeschiedenis, behandelgeschiedenis, precipiterende en predisponerende factoren, biografie en sociaalmaatschappelijk functioneren. Het proces begint al voor het eerste gesprek. De verwijsbrief en eerste indruk geven al richting aan het beeld dat gevormd wordt.
Waarop is het eerste gesprek gericht?
In het eerste gesprek is gericht op het perspectief van de cliënt. Ondertussen heeft het eerste gesprek vier doelen: een voorlopige diagnose stellen, informatie over de gang van zaken geven, toestemming van de cliënt vragen, en het motiveren voor behandeling. Het is de bedoeling dat de intaker meerdere alternatieven voor behandeling biedt en niet alleen vanuit het eigen psychotherapeutisch kader denkt.
Hoe verschillen beschrijvende en verklarende diagnostiek?
Het is belangrijk om onderscheid te maken tussen beschrijvende en verklarende diagnostiek. Elk geeft antwoord op een ander soort vraagstelling. Bij beschrijvende diagnostiek gaat het om het beschrijven van klachten en symptomen, en het geven van een classificatie volgens de DSM. De DSM-classificatie is vereist voor de zorgverzekering en bovendien zijn er diagnosebehandelingcombinaties (DBC’s) aan gekoppeld. Bij verklarende diagnostiek gaat het om factoren die de klachten hebben doen ontstaan en die ze in stand houden. Bij de besluitvorming over de behandeling wordt rekening gehouden met deze factoren. Er zijn verschillende stromingen die gebruikt worden bij het verklaren van psychopathologie. Ten eerste is er het psychodynamisch kader. Hierbij wordt gericht op de dynamiek van het ego, zoals realiteitstoetsing, impulscontrole en afweermechanismen. Ten tweede kan het cognitief gedragstherapeutisch kader gebruikt worden. In dit kader staan automatische gedachten, leefregels en diepgewortelde schema’s centraal. Hieruit kan het gedrag van de cliënt verklaard worden. Tot slot is er het systeemkader. Hierin gaat het om relaties tussen personen en hun interacties. Ook de context waarin personen zich bevinden wordt meegenomen. Zo ontstaan circulaire verbanden, waarin sprake is van wederkerige invloed. Welk kader gebruikt wordt voor de verklaring, kan van grote invloed zijn op de behandelingskeuze en de vraag is of die behandeling dan adequaat is. Daarom is het van groot belang dat de verklarende diagnostiek zoveel mogelijk vanuit wetenschappelijke inzichten wordt gedaan.
Wat omvat doelgerichte en evidence-based diagnostiek?
Het is ook belangrijk dat de diagnostiek doelgericht is. Er kan onderscheid gemaakt worden tussen de volgende doelen: classificatie en diagnose, screening, prognose, behandelingskeuze, evaluatie van behandelingsverloop, en evaluatie van behandelingseffect. Het doel is sturend in de keuze voor diagnostische methoden: de manier waarop informatie wordt verzameld. Welke diagnostische methoden bruikbaar zijn, kan bepaald worden aan de hand van wetenschappelijk onderzoek. Evidence-based behandelingen zijn al langer gangbaar, maar sinds kort is de vraag naar evidence-based diagnostiek ook steeds groter. Helaas zijn veel instrumenten nog niet goed onderzocht of nog niet geschikt vanwege te lage betrouwbaarheid en validiteit. Bij het trekken van conclusies moet daarom kritisch geïnterpreteerd worden, door goed te kijken naar meetpretentie en normgroepen.
