Wanneer ben je verslaafd?
- Als je meer nodig hebt voor hetzelfde effect (tolerantie)
- Als je de stof nodig hebt voor het herstellen van de oorspronkelijke toestand
- Onthoudingsverschijnselen (receptoren worden minder gevoelig met herhaald gebruik)
- Tolerantie + onthoudingsverschijnselen = afhankelijkheid
Heroïne: tolerantie + fysieke afhankelijkheid.
Cocaïne: geen fysieke afhankelijkheid maar wel verslavend, dus een andere soort afhankelijkheid.
Afhankelijkheid kan leiden tot substance-related disorders, bijvoorbeeld alcohol use disorder.
Beloningssysteem (liking) en dwang-systeem (wanting) zorgen samen voor een verslaving (nucleus accumbens + ventral tegmental area).
Cocaïne is een her-opname remmer, waardoor dopamine langer in de synapsspleet blijft. Dit heeft een presynaptisch effect op de afgegeven neurotransmitter: remt activiteit transporter-eiwit. Daardoor is er meer beschikbare dopamine, vooral in nucleus accumbens.
Alle verslavende stoffen activeren het beloningssysteem (zelf-toediening), maar verschillende stoffen reageren primair met verschillende chemische systemen in het brein (neurotransmitter-systemen).
Acuut effect van alcohol: GABA werkt kalmerend / sederend en opioïde-receptor zorgt voor euforie.
Een mindere sedatie zorgt voor een grotere kans op misbruik, dit verschilt per individu.
Dwang systeem (wanting): zelfs wanneer ‘liking’ er niet meer is/verminderd is, het alsnog willen bemachtigen en innemen van drugs. Dit berust op overmatige activiteit van hersencircuits die verschillen van die waarop liking berust. Dit is vergelijkbaar met obsessief-compulsieve stoornis. Wanting gaat samen met excessieve activiteit in apart hersencircuit (het zelfde hersencircuit als bij dwang verslaving).
Liking response wordt bevorderd door cocaïne of voedsel in de Nucleus Accumbens van een rat. Neuronen in de basale ganglia of striatum bevorderen wanting responsen, zonder dat liking optreed.
Overgang van liking naar wanting gebeurt door afnemende waardering voor de drug (VTA --> NAcc) en toenemende gevoeligheid voor conditionele stimuli uiteindelijk (VTA --> BG).
De verslaving wordt onderhouden door onder andere cravings, cues en conditionering.
Met Varenicline, GABA-antagonisten en opioïde-receptor agonist kan je betrokken receptoren (gedeeltelijk) stimuleren. Met straffen kan je operante conditionering toepassen om wanting-gedrag uit te laten doven.
Bij overgevoelige receptoren na gebruik van nicotine: gebruik varenicline. Dit bezet de bindingsplekken en zorgt voor hetzelfde (maar kleiner) effect.
