Een beroerte wordt gezien als een plotseling optredende focale uitval in de hersenen als gevolg van ischemie of een spontane intracerebrale bloeding. Er is sprake van een beroerte wanneer focale uitvalsverschijnselen aanwezig zijn die acuut op zonder voortekenen zijn begonnen, binnen vijf minuten ontwikkeld hebben en te verklaren zijn vanuit een stoornis in één cerebraal stroomgebied.
Anamnese bij beroerte
Bij verdenking op een beroerte, moet de FAST-test uitgevoerd worden (face, arm, speech, time). De tanden moeten worden laten zien, beide armen moeten naar voren horizontaal gestrekt worden waarbij de handpalmen naar boven gedraaid moeten worden. Verder moet gevraagd worden of er veranderingen zijn opgetreden in de spraak en er moet gevraagd worden vanaf wanneer de klachten zijn begonnen. Verder moet gevraagd worden naar de aard van de uitvalsverschijnselen, het tijdstip van ontstaan, een acuut of geleidelijk begin, voortekenen, beloop, eerdere episoden en het gebruik van anticoagulantia en risicofactoren. Risicofactoren zijn overmatig alcoholgebruik, pijn op de borst, onregelmatige hartslag, roken, migraine met aura, hoge bloeddruk, hart- en vaatziekten, diabetes, obesitas, drugsgebruik en gebruik van systemische hormonale medicatie.
Lichamelijk onderzoek
Het lichamelijk onderzoek bestaat uit het beoordelen van de klinische toestand, de stabiliteit en het bewustzijn. Ook moet de ABCDE worden nagegaan. De bloeddruk moet gemeten worden en het hart moet geausculteerd worden. Ook moet de bloedglucosewaarde worden bepaald. De aanwezigheid, ernst en aard van de uitvalsverschijnselen moet worden uitgezocht.
Beleid
Het beleid bij een beroerte is vooral gericht op revalidatie. Er moet besproken worden wat de beperkingen zijn, of de patiënt voldoende steun ervaart, andere problemen en een revalidatieprogramma. Communicatie en waarneming, maar ook de aanwezigheid van cognitieve stoornissen en belastbaarheid van de mantelzorg moeten worden besproken. Leefstijlinterventies zijn ook erg belangrijk ter preventie van een nieuwe beroerte. Nadat iemand een beroerte heeft doorgemaakt, moet levenslang behandeld worden met acetylsalicylzuur, dipyridamol, simvastatine en antihypertensieve medicatie.
