SOLK

Somatisch onvoldoende verklaarde lichamelijke klachten worden gekenmerkt door lichamelijke klachten die langer dan enkele weken duren en als er geen aandoening wordt gevonden bij adequaat medisch onderzoek. Milde SOLK bestaat uit een lichte functionele belemmering en één of enkele SOLK-klachten binnen één tot twee klachtenclusters. Een matig-ernstige SOLK bestaat uit functionele belemmeringen en meerdere SOLK-klachten in ten minste drie clusters en/of de klacht duurt langer dan verwacht, afhankelijk van het normale beloop van de betreffende klacht. Ernstige SOLK tenslotte bestaat uit ernstige functionele belemmeringen, SOLK-klachten in alle clusters, klachtenduur langer dan drie maanden. De klachtenclusters zijn:

  • Gastro-intestinaal

  • Cardiopulmonaal

  • Bewegingsapparaat

  • Algemeen aspecifiek: moeheid, hoofdpijn, duizeligheid, concentratie/geheugenklachten

Verder kan SOLK gecombineerd gediagnosticeerd worden met een angststoornis of depressie als de nadruk sterker ligt bij lichamelijke klachten dan bij de psychische klachten, de lichamelijke klachten eerder aanwezig zijn dan de psychische en wanneer beide een aparte behandeling vereisen.

Anamnese

De anamnese bij SOLK bestaat uit het uitvragen van de klacht, veel stiltes laten vallen en de hulpvraag van de patiënt moet goed duidelijk worden. Verder is het belangrijk om de SCEGS uit te vragen, namelijk somatisch, cognitief, emotioneel, gedragsmatig en sociaal.

Wanneer lichamelijk onderzoek en aanvullend onderzoek uitgevoerd gaat worden, moet expliciet de reden daarvoor goed besproken worden.

Beleid

De huisarts zelf kan zorgen voor afsluiting van de klachtexploratie en eventueel lichamelijk of aanvullend onderzoek uitvoeren. Vervolgens wordt een probleemdefinitie gezamenlijk opgesteld op basis van de klachten. Er wordt voorlichting en uitleg gegeven, herstelbelemmerende factoren worden besproken en advisering vindt plaats. Vervolgens wordt gezamenlijk een tijdcontingent plan opgesteld en controle van de voortgang en stagnering. In de tweede stap is de SOLK matig-ernstig, waardoor verwijzing naar de psychosomatische fysiotherapeut of oefentherapeut kan plaatsvinden, evenals ondersteuning van de POH, of GGZ. Tenslotte kunnen mensen met ernstige SOLK nog verder verwezen worden naar een multidisciplinair team of behandelcentrum.

  2. Use the topics and taxonomy terms
    • The topics and taxonomy of the study and working fields gives you insight in the amount of summaries that are tagged by authors on specific subjects. This type of navigation can help find summaries that you could have missed when just using the search tools. Tags are organised per field of study and per study institution. Note: not all content is tagged thoroughly, so when this approach doesn't give the results you were looking for, please check the search tool as back up
  3. Check or follow your (study) organizations:
    • by checking or using your study organizations you are likely to discover all relevant study materials.
    • this option is only available trough partner organizations
  4. Check or follow authors or other WorldSupporters
    • by following individual users, authors  you are likely to discover more relevant study materials.
  5. Use the Search tools
    • 'Quick & Easy'- not very elegant but the fastest way to find a specific summary of a book or study assistance with a specific course or subject.
