NHG-standaard (Geneeskunde): Diverticulitis - M99
Diverticulitis is een ontsteking van één of meerdere divertikels van het colon. Gecompliceerde diverticulitis is een ontsteking van één of meerdere divertikels van het colon in combinatie met complicaties. Een divertikelbloeding is een bloeding uit een divertikel die eventueel ook al ontstoken was. Diverticulose is de aanwezigheid van divertikels in het colon, zonder daarbij klachten te hebben. Ongecompliceerde diverticulitis wordt gekenmerkt door binnen enkel dagen ontstane aanhoudende scherpe, stekende pijn links onder in de buik, druk en loslaatpijn linksonder en afwezigheid van alarmsignalen. Gecompliceerde diverticulitis wordt gekenmerkt door tekenen van peritoneale prikkeling, ileus, rectaal bloedverlies, hypotensie, lokaal palpabele weerstand en een sterk verhoogde CRP.
Anamnese
Bij verdenking op diverticulitis moet gevraagd worden naar de aard, het patroon en de duur van de buikpijn, invloed van bewegen op pijn, koorts, misselijkheid, het defecatiepatroon, de voorgeschiedenis, chronische ziekten en medicatiegebruik.
Lichamelijk onderzoek
Het lichamelijk onderzoek bij een verdenking op diverticulitis bestaat uit het beoordelen van de algemene indruk, de bloeddruk en pols en temperatuur meten, onderzoek van het abdomen en een vaginaal toucher en rectaal toucher op indicatie.
Aanvullend onderzoek
Het aanvullend onderzoek bij een verdenking op diverticulitis bestaat uit het bepalen van de CRP-waarde en urineonderzoek.
Behandeling van diverticulitis
De behandeling van diverticulitis bestaat uit voorlichting over de aandoening. De diverticulitis geneest vaak vanzelf, maar kan recidiveren. De temperatuur moet dagelijks gemeten worden en er moet direct contact gezocht worden met de huisarts bij rectaal bloedverlies, toename van de klachten, koorts hoger dan 39 graden en braken. De medicamenteuze behandeling bestaat uit pijnstilling, zoals paracetamol. Wanneer obstipatie aanwezig is, kunnen laxantia worden geadviseerd.
