De Zelf-Inventarisatielijst (ZIL) is een vragenlijst die wordt gebruikt om de symptomen van posttraumatische stressstoornis (PTSS) te beoordelen. De lijst is gebaseerd op de criteria van de DSM-IV en omvat 16 items verdeeld over drie subschalen:
- Herbeleving: Ervaart de persoon flashbacks of nachtmerries over het trauma?
- Vermijding: Vermijdt de persoon situaties of herinneringen aan het trauma?
- Hyperarousal: Is de persoon prikkelbaar, slaapt hij of zij slecht en heeft hij of zij last van concentratieproblemen en woede-uitbarstingen?
De ZIL wordt ingevuld door de cliënt zelf. De scores op de items worden bij elkaar opgeteld om een totaalscore te krijgen. Een score van 51 of hoger duidt op een mogelijke PTSS-diagnose.
Wat zijn de belangrijkste kenmerken van de ZIL?
- Gebaseerd op DSM-IV criteria: De ZIL is ontworpen om de symptomen van PTSS te meten in overeenstemming met de diagnostische criteria van de DSM-IV.
- Drie subschalen: De lijst omvat 16 items verdeeld over drie subschalen: Herbeleving, Vermijding en Hyperarousal, die elk aspect van PTSS belichten.
- Totaalscore en subschaalscores: Naast een totaalscore die een indicatie geeft voor de algehele ernst van PTSS, biedt de ZIL ook afzonderlijke scores voor elk van de drie subschalen, wat een meer gedetailleerd beeld geeft van de symptomatologie.
- Gebruiksvriendelijk: De ZIL is eenvoudig in te vullen en te interpreteren, waardoor het een toegankelijk instrument is voor zowel cliënten als professionals.
- Betrouwbaar en valide: De ZIL is uitgebreid onderzocht en bewezen betrouwbaar en valide te zijn in het meten van PTSS-symptomen.
Waarom is de ZIL belangrijk?
De ZIL is een belangrijk instrument om verschillende redenen:
- Snelle screening: De ZIL kan snel en eenvoudig worden ingezet om PTSS te screenen in een breed scala aan settings, zoals huisartsenpraktijken, GGZ-instellingen, hulporganisaties en crisisondersteuningsteams.
- Beoordeling van ernst: De ZIL helpt bij het bepalen van de ernst van PTSS-symptomen, wat belangrijk is voor het bepalen van de juiste behandeling en het monitoren van de voortgang.
- Evaluatie van behandelresultaten: De ZIL kan herhaaldelijk worden afgenomen om te evalueren of PTSS-behandeling effectief is en of symptomen verminderen.
- Onderzoeksdoeleinden: De ZIL wordt ook gebruikt in onderzoek naar PTSS om de symptomatologie in verschillende populaties te bestuderen en de effectiviteit van behandelingen te evalueren.
Hoe wordt de ZIL in de praktijk toegepast?
De ZIL wordt breed toegepast in diverse settings:
- GGZ-praktijken: Psychologen en psychiaters gebruiken de ZIL om PTSS te diagnosticeren, de ernst van symptomen te bepalen en de voortgang van de behandeling te monitoren.
- Huisartsenpraktijken: Huisartsen kunnen de ZIL gebruiken om PTSS te screenen bij patiënten die mogelijk een trauma hebben meegemaakt.
- Hulporganisaties: Organisaties die slachtoffers van trauma's ondersteunen, kunnen de ZIL gebruiken om hun cliënten te screenen op PTSS en passende hulp te bieden.
- Onderzoeksinstituten: Onderzoekers gebruiken de ZIL om de symptomatologie van PTSS in verschillende populaties te bestuderen en de effectiviteit van behandelingen te evalueren.
Praktijkvoorbeeld
Een vrouw van 28 jaar bezoekt haar huisarts na een auto-ongeluk waarbij ze ernstig gewond is geraakt. Ze ervaart flashbacks, nachtmerries, vermijdt autorijden en heeft moeite met slapen en concentreren. De huisarts laat haar de ZIL invullen en de score is 58, wat duidt op een mogelijke PTSS-diagnose. De huisarts verwijst de vrouw door naar een psycholoog voor verdere diagnostiek en behandeling.
Kritische kanttekeningen
Ondanks de waarde van de ZIL, zijn er enkele aandachtspunten:
- Subjectieve vragenlijst: De ZIL is gebaseerd op zelfrapportage, waardoor scores beïnvloed kunnen worden door factoren zoals stemming, coping en sociaal-culturele context.
- Meet niet alle PTSS-symptomen: De ZIL richt zich op de kern symptomen van PTSS, maar meet niet alle mogelijke symptomen, zoals dissociatieve symptomen, cognitieve problemen en veranderingen in emoties en gedrag.
- Geen vervanging voor klinische expertise: De ZIL is een hulpmiddel bij screening, diagnose en evaluatie, maar mag niet worden gebruikt als vervanging voor de expertise van een gekwalificeerde professional in de geestelijke gezondheidszorg.
