Er wordt steeds meer gebruik gemaakt van EST’s: empirically supported treatments. Dit zijn therapeutische interventies die gemaakt zijn voor specifieke psychische stoornissen of problemen. De kritiek op de evidence-based interventies voor specifieke problemen is dat uit onderzoek blijkt dat specifieke methodiek maar voor 15% verantwoordelijk is voor de uitkomst van een interventie. Belangrijker zijn algemeen werkende factoren, met name variabelen die te maken hebben met de relatie tussen hulpverlener en cliënt. Deze relatie-variabelen zijn vooral bij jongeren en kinderen nog weinig onderzocht. Bij jongeren zijn deze variabelen zeker belangrijk omdat jonge kinderen en adolescenten vaak gestuurd worden door hun ouders en dus niet altijd vrijwillig aan een interventie meedoen.
Uit onderzoek blijkt ook dat deze variabelen een gemiddeld tot groot effect hebben op de resultaten van de interventie. De directe beïnvloedingsvaardigheden van de hulpverlener en de relatie tussen hulpverlener en cliënt waren vooral belangrijk. De directe beïnvloedingsvaardigheden van de hulpverlener zijn de vaardigheden om duidelijk uit te leggen waarom een bepaalde behandeling wordt ingezet. Hierdoor krijgt een cliënt vertrouwen in de hulpverlener en positievere verwachtingen over de uitkomst van de interventie. Wat ook belangrijk is gebleken, is de bereidheid van ouders en kind om mee te doen aan een interventie, evenals de daadwerkelijke participatie. Hierbij is echter belangrijk dat de interventie ook zinvol is. Ouders hebben bij (jonge) kinderen nog veel invloed op het dagelijks leven en wat een kind denkt of voelt, hierdoor oefenen zij invloed uit op de motivatie van het kind, maar bepalen zij ook de mate waarin een stimulerende omgeving gefaciliteerd wordt. Opvallend is dat uit onderzoek blijkt dat de therapeutische relatie belangrijker is dan de therapeutische alliantie (samenwerkingsverband). Dit lijkt vreemd omdat de twee begrippen veel op elkaar lijken en de verwachting ook was dat zij beiden evenveel invloed zouden uitoefenen. Dit verschil kan verklaard worden door de meting met verschillende testinstrumenten in onderzoek naar deze twee begrippen.
Karver et al. (2005) hebben een therapeutisch proces model bedacht, waarin relatievariabelen elkaar beïnvloeden en zo ook de resultaten van de interventie beïnvloeden. Zo beïnvloedt bijvoorbeeld de persoonlijkheid en het gedrag van de hulpverlener de cognitieve en affectieve reacties van de cliënt. Deze reacties bepalen weer de mate van participatie aan de interventie, en daarmee het resultaat van de interventie. Dit proces van Karver wordt ondersteund door onderzoek.
