De PCL-5 (PTSS Checklist voor DSM-5) is een veelgebruikte vragenlijst om de symptomen van posttraumatische stressstoornis (PTSS) te beoordelen. De lijst is gebaseerd op de criteria van de DSM-5, de nieuwste editie van het diagnostisch handboek voor psychische stoornissen van de American Psychiatric Association (APA).
Wat zijn de belangrijkste kenmerken van de PCL-5?
- 20 items: De PCL-5 bevat 20 vragen die betrekking hebben op de kern symptomen van PTSS, verdeeld over 5 subschalen:
- Herbeleving: Ervaart de persoon flashbacks of nachtmerries over het trauma?
- Vermijding: Vermijdt de persoon situaties of herinneringen aan het trauma?
- Negatieve cognities en stemmingen: Heeft de persoon negatieve gedachten en gevoelens over zichzelf en de wereld?
- Verhoogde prikkelbaarheid: Is de persoon prikkelbaar of schrikachtig?
- Schuldgevoelens en schaamte: Ervaart de persoon schuldgevoelens of schaamte over het trauma?
- Totaalscore: De scores op de items worden bij elkaar opgeteld om een totaalscore te krijgen. Een score van 31 of hoger duidt op een mogelijke PTSS-diagnose.
- Normering: De PCL-5 is genormeerd voor de algemene bevolking en voor specifieke subgroepen, zoals trauma-gerelateerde hulpverleners en veteranen.
- Zelfrapportage: De PCL-5 wordt ingevuld door de cliënt zelf, waardoor het een efficiënte tool is voor screening en diagnostiek.
Waarom is de PCL-5 belangrijk?
De PCL-5 is een belangrijk instrument om verschillende redenen:
- Snelle screening: De PCL-5 kan snel en eenvoudig worden ingezet om PTSS te screenen in diverse settings, zoals huisartsenpraktijken, GGZ-instellingen, hulporganisaties en crisisondersteuningsteams.
- Beoordeling van ernst: De PCL-5 helpt bij het bepalen van de ernst van PTSS-symptomen, wat belangrijk is voor het bepalen van de juiste behandeling en het monitoren van de voortgang.
- Evaluatie van behandelresultaten: De PCL-5 kan herhaaldelijk worden afgenomen om te evalueren of PTSS-behandeling effectief is en of symptomen verminderen.

Wat zijn toepassingen van de PCL-5 in de praktijk?
De PCL-5 wordt breed toegepast in diverse settings:
- GGZ-praktijken: Psychologen en psychiaters gebruiken de PCL-5 om PTSS te diagnosticeren, de ernst van symptomen te bepalen en de voortgang van de behandeling te monitoren.
- Huisartsenpraktijken: Huisartsen kunnen de PCL-5 gebruiken om PTSS te screenen bij patiënten die mogelijk een trauma hebben meegemaakt.
- Hulporganisaties: Organisaties die slachtoffers van trauma's ondersteunen, kunnen de PCL-5 gebruiken om hun cliënten te screenen op PTSS en passende hulp te bieden.

Praktijkvoorbeeld
Een man van 35 jaar bezoekt zijn psycholoog na een gewapende overval in zijn huis. Hij ervaart flashbacks, nachtmerries, vermijdt de buurt waar de overval plaatsvond, heeft moeite met concentreren en is prikkelbaar. De psycholoog laat hem de PCL-5 invullen en de score is 42, wat duidt op een PTSS-diagnose. De psycholoog start met traumagerichte therapie om de symptomen van de man te verminderen.
Kritische kanttekeningen
Ondanks de waarde van de PCL-5, zijn er enkele aandachtspunten:
- Meet niet alle PTSS-symptomen: De PCL-5 richt zich op de kern symptomen van PTSS, maar meet niet alle mogelijke symptomen, zoals dissociatieve symptomen, veranderingen in emoties en gedrag, en problemen in intermenselijke relaties.
- Geen vervanging voor klinische expertise: De PCL-5 is een hulpmiddel bij screening, diagnose en evaluatie, maar mag niet worden gebruikt als vervanging voor de expertise van een gekwalificeerde professional in de geestelijke gezondheidszorg.
