Insomnie, ook wel slapeloosheid genoemd, is een veelvoorkomende slaapstoornis die gekenmerkt wordt door moeite met in slaap vallen, doorslapen of te vroeg wakker worden. Mensen met insomnie ervaren vaak een slechte slaapkwaliteit, wat leidt tot vermoeidheid, prikkelbaarheid en concentratieproblemen overdag.
Wat zijn de belangrijkste symptomen van insomnie?
- Moeite met in slaap vallen (langer dan 30 minuten)
- Vaak wakker worden tijdens de nacht
- Te vroeg wakker worden en niet meer in slaap kunnen vallen
- Onverfrissende slaap
- Slaperigheid overdag
- Prikkelbaarheid, humeurigheid en concentratieproblemen
- Verminderde energie en motivatie
Waarom is insomnie belangrijk?
Slaap is essentieel voor onze lichamelijke en geestelijke gezondheid. Onvoldoende of verstoorde slaap kan leiden tot diverse problemen, zoals:
- Verhoogd risico op lichamelijke aandoeningen: Hart- en vaatziekten, diabetes, beroerte, obesitas en een verzwakt immuunsysteem.
- Psychische problemen: Depressie, angststoornissen, verslavingsproblemen en verminderde cognitieve functies.
- Verminderde kwaliteit van leven: Verminderde energie, concentratieproblemen, prikkelbaarheid en moeite met het omgaan met stress.
Wat is de prevalentie in Nederland?
In Nederland kampt ongeveer 10% van de volwassenen met chronische insomnie. Dit komt neer op zo'n 1,7 miljoen mensen. Ongeveer 25% van de volwassenen ervaart op enig moment in hun leven symptomen van insomnie. De prevalentie neemt toe met de leeftijd.
Hoe werkt insomnie in de praktijk?
Insomnie kan verschillende oorzaken hebben, waaronder:
- Stress en piekeren: Overmatige zorgen of piekeren over dagelijkse problemen of toekomstige gebeurtenissen kunnen het moeilijk maken om in slaap te vallen of doorslapen.
- Slaaphygiëne: Ongezonde slaapgewoonten, zoals onregelmatige slaaptijden, overdag dutjes doen of te veel cafeïne of alcohol gebruiken, kunnen insomnie verergeren.
- Medische aandoeningen: Bepaalde medische aandoeningen, zoals pijnklachten, astma, refluxziekte en hormoonstoornissen, kunnen slaapstoornissen veroorzaken.
- Medicatie: Sommige medicijnen kunnen als bijwerking slapeloosheid veroorzaken.
- Psychische aandoeningen: Depressie, angststoornissen en posttraumatische stressstoornis (PTSS) komen vaak voor in combinatie met insomnie.
Praktijkvoorbeeld
Een student heeft al wekenlang moeite met slapen. Ze piekert veel over haar studie en heeft last van examenstress. Ze valt vaak pas laat in slaap en wordt 's nachts vaak wakker. Overdag is ze moe en heeft ze moeite met concentreren. Ze besluit naar de huisarts te gaan, die haar slaapklachten bespreekt en lichamelijke oorzaken uitsluit. De huisarts adviseert de student om haar slappatroon te reguleren, ontspanningstechnieken te proberen en dagboeken bij te houden om haar piekergedachten te structureren. In overleg met de student beslist de huisarts om slaapmedicatie voor te schrijven voor de korte termijn, om de vicieuze cirkel van slapeloosheid te doorbreken.
Wat is de behandeling van insomnie?
De behandeling van insomnie hangt af van de oorzaak. In veel gevallen is een combinatie van verschillende behandelingen nodig:
- Slaaphygiëne: Verbeteren van slaapgewoonten, zoals regelmatige slaaptijden aanhouden, een ontspannen bedtijdroutine creëren, overdag voldoende daglicht en beweging krijgen en cafeïne en alcohol vermijden voor het slapengaan.
- Cognitieve gedragstherapie (CGT-I): Een effectieve psychologische behandeling gericht op het identificeren en veranderen van negatieve gedachten en gedragingen die de slaap beïnvloeden.
- Ontspanningstechnieken: Mindfulness, meditatie, yoga en ademhalingsoefeningen kunnen stress verminderen en slaap bevorderen.
- Slaapmedicatie: In sommige gevallen kan slaapmedicatie voor de korte termijn worden voorgeschreven om de slaap te verbeteren. Dit dient altijd te gebeuren in overleg met een arts.
- Behandeling van onderliggende aandoeningen: Medicatie of therapie voor medische aandoeningen of psychische problemen die de insomnie veroorzaken of in stand houden, kan de slaapkwaliteit verbeteren.

- Zelfzorg: Regelmatige lichaamsbeweging, een gezond voedingspatroon en stressmanagement kunnen de algehele gezondheid en slaapkwaliteit verbeteren.
- Professionele hulp: Raadpleeg een arts of slaapspecialist als je langdurig last hebt van insomnie, ondanks het toepassen van bovenstaande tips.
- Geduld: Het kan tijd duren voordat de behandeling van insomnie effect heeft. Blijf gemotiveerd en volg de adviezen van je arts of therapeut op.
