De Yale-Brown Obsessive Compulsive Scale (YBOCS) is een gestructureerd klinisch interview dat wordt gebruikt om de ernst van obsessieve-compulsieve stoornis (OCS) te meten. Hoe wordt de Yale-Brown Obsessive Compulsive Scale in de praktijk toegepast?
De YBOCS wordt meestal afgenomen in een klinische setting, zoals een polikliniek voor geestelijke gezondheidszorg of een ziekenhuis. De interviewer stelt de patiënt een reeks gestandaardiseerde vragen over zijn of haar obsessieve gedachten en dwanghandelingen. De patiënt beoordeelt vervolgens de ernst van zijn of haar symptomen op een schaal van 0 tot 4.
Praktijkvoorbeeld
Een 30-jarige vrouw komt bij haar huisarts met klachten over obsessieve gedachten over vuil en besmetting. Ze heeft ook last van dwanghandelingen, zoals herhaaldelijk haar handen wassen en controleren of de deuren op slot zijn. De huisarts vermoedt dat de vrouw OCS heeft en verwijst haar naar een psychiater voor nader onderzoek. De psychiater neemt de YBOCS af bij de vrouw en de resultaten bevestigen de diagnose OCS met een score van 25. De psychiater bespreekt met de vrouw de verschillende behandelmogelijkheden en stelt een behandelplan op dat cognitieve gedragstherapie (CBT) en medicatie omvat. De YBOCS wordt regelmatig afgenomen bij de vrouw om haar voortgang te volgen en de behandeling indien nodig aan te passen.
Kritische kanttekeningen
Hoewel de YBOCS een waardevol hulpmiddel is, zijn er enkele kritische kanttekeningen waarmee rekening moet worden gehouden:
- De YBOCS is tijdrovend: Het afnemen van de YBOCS kan 30 minuten of langer duren.
- De YBOCS is subjectief: De antwoorden op de vragen van de YBOCS zijn subjectief en kunnen beïnvloed worden door de stemming en motivatie van de patiënt.
- De YBOCS is niet geschikt voor alle patiënten: De YBOCS is niet geschikt voor patiënten met een co-morbide psychiatrische stoornis of voor patiënten met een verstandelijke beperking.
