Sepsissterfte - Zanten, Arbous, Brinkman - Artikel
Er zijn in Nederland 14.000 patiënten die met sepsis opgenomen worden in het ziekenhuis. Het is een gegeneraliseerde ontstekingsreactie die wordt veroorzaakt door een infectie. Wanneer de sepsis als ernstig wordt ingedeeld, is er sprake van uitval van orgaanfuncties. Het aantal sterftegevallen in het ziekenhuis in Nederland als gevolg van sepsis werd geregistreerd aan de hand van de APACHE-score. APACHE staat voor “acute physiology and chronic health evaluation”.
Het bleek dat de ziekenhuis sterfte aan de hand van deze score met 16,7 procent daalde over 3,5 jaar. Gedurende deze 3,5 jaar gingen veel meer ziekenhuizen zich aan de richtlijnen voor sepsis houden. De sterftedaling werd niet veroorzaakt door minder zieke patiënten. Wanneer mensen behandeld worden aan de hand van protocollen en bundels zorgt dit voor een snelle, tijdige en uniforme werkwijze, waardoor de variaties in de klinische praktijk afnemen.
Het is van belang dat er goed samengewerkt wordt tussen medici en verpleegkundigen bij de behandeling van sepsis. Wanneer iemand een ernstige sepsis heeft doorgemaakt, is de kans op sterfte in de eerste drie jaar alsnog aanwezig, waardoor goed gekeken moet worden naar de lange termijn effecten. Bovendien is het zo dat er een toename heeft plaatsgevonden van patiënten die na ontslag van de IC-afdeling in een revalidatiecentrum of verpleeghuis terecht zijn gekomen. Hieruit blijkt dat niet alleen de overleving van de sepsis van belang is, maar ook het toekomstbeeld en de kwaliteit van leven. Het is van belang om prestaties te blijven meten en wetenschappelijk onderbouwde sepsisindicatoren te ontwikkelen. Hierdoor kunnen ziekenhuizen onderling hun prestaties met elkaar vergelijken. Bovendien is het van essentieel belang dat sepsis snel herkent en behandeld wordt. Tenslotte is meer onderzoek nodig naar de consequenties van de sepsis op lange termijn.
