Waarom is diagnostisch onderzoek belangrijk in de kinder- en jeugdpsychologie?
In de kinder- en jeugdpsychologie wordt verondersteld dat uitvoerig diagnostisch onderzoek noodzakelijk is voor een goede behandeling. Hierbij wordt vaak aan twijfelachtige aannames vastgehouden. De eerste aanname is dat oorzaken van psychopathologie zijn vast te stellen. Dit is echter lastig: een classificatie volgens de DSM is niet meteen een diagnose, en daar kunnen ook niet goed oorzaken aan verbonden worden. Twee dingen worden vaak aan elkaar gerelateerd, en dan is het meteen een oorzaak. Dit is niet zo. Ten tweede, wordt aangenomen dat het gedrag tijdens psychiatrisch onderzoek representatief is voor gedrag in een andere context. Er wordt tijdens het psychiatrisch onderzoek veel gebruik gemaakt van observatie. Uit onderzoek is echter bekend dat gedrag in de context van een PO sterk kan verschillen van gedrag daarbuiten. Observatiegegevens moeten daarom voorzichtig geïnterpreteerd worden. Een andere denkfout is dat meetinstrumenten een juist beeld geven van de persoonlijkheid. Het voorspellend vermogen van testen is gering. Ook zijn er vaak geen normgegevens aanwezig. Beter is om tests te gebruiken om de ernst van de problematiek en therapie-effecten in kaart te brengen. Daarnaast is een aanname dat het bekend is hoe een ‘normaal’ kind zich gedraagt: dit weten we niet. Het ideale gezin bestaat vermoedelijk niet.
Nog een aanname is dat uitgebreide diagnostiek ervoor zorgt dat de inhoud van de behandeling duidelijk is. We hebben niet zulke specifieke behandelingsstrategieën, dat deze een antwoord kunnen zijn op alle bijzonderheden die in het diagnostisch onderzoek zijn vastgesteld. Bovendien worden de gegevens die tijdens de diagnostiek verzameld zijn lang niet altijd gebruikt om de uiteindelijke behandeling te selecteren. De uitvoerige diagnostiek is dan dus eigenlijk nauwelijks zinvol geweest. Behandeling kan pas plaatsvinden als de oorzaken van de stoornis bekend zijn, is een volgende aanname. Schulte en collega’s deden een onderzoek waarbij patiënten één van drie behandelingen kregen: 1) een standaardbehandeling, 2) een behandeling op maat of 3) de behandeling op maat van een andere patiënt. De uitkomst was dat de standaardbehandeling significant meer effect had dan de andere twee groepen, waar geen verschil tussen werd gevonden. Een andere onjuiste aanname is dat wanneer een behandeling mislukt, de diagnose niet goed was. Het probleem ligt vaak juist bij de behandelingsstrategie zelf. Er moeten meer effectieve behandelingen gebruikt worden.
Conclusie
De Haan concludeert dat we op moeten houden zoveel tijd te besteden aan diagnostisch onderzoek. We moeten meer nadruk leggen op het ontwikkelen van werkzame behandelingsstrategieën. Zoeken naar oorzaken van stoornissen moeten we aan onderzoekers overlaten. Clinici en behandelaren zijn slechte onderzoekers. We moeten ons minder richtingen bij de intake op info die met de oorzaak te maken heeft, meer richten op de behandeling.
