Waar gaat dit artikel over?
De behandeling van zedendelinquentie is het meest effectief wanneer deze is aangepast op de risico-behoefte-responsiviteit (RNR) principes, waarbij er vanuit wordt gegaan dat het niveau van behandeling afgestemd dient te zijn op het risiconiveau. Met andere woorden: "hoge-risico" zedendelinquenten dienen meer intensieve behandeling te ontvangen dan "lage-risico" zedendelinquenten, om recidivisme (herhaling) te voorkomen. Dit risiconiveau wordt in kaart gebracht met gestructureerde instrumenten voor risicobeoordeling (structured risk assessment instruments). In dit artikel worden de psychometrische aspecten (1. voorspellende waardes; 2. missende waardes; 3. tussen-beoordelaars-betrouwbaarheid) besproken van de volgende negen instrumenten:
- Rapid Risk Assessment for Sexuel Offense Recidivism (RRASOR)
- Static-99
- Static-99R (revised)
- Static-2002 (met een kleine aanpassing)
- Static-2002R (revised)
- Structured Anchored Clinical Judgements Minimum (SACJ)

Welke instrumenten zijn geschikt voor een accurate inschatting van het risico op (recidive van) zedendelinquentie?
De negen hiervoor genoemde instrumenten werden in deze studie beoordeeld aan de hand van een 5- en 10-jarige follow-up periode. Er is gebruik gemaakt van twee steekproeven van Nederlandse mannen, die veroordeeld zijn vanwege zedendelinquentie (aanranding, verkrachting, misbruik). De eerste steekproef bestond uit 96 mannen die werden ontslagen uit intramurale behandeling tussen 1996 en 2002. De tweede steekproef bestond uit een random selectie van 25% van mannen die zijn vrijgelaten uit de gevangenis tussen 1996 en 2002. Bij de tweede steekproef gaat het om 341 mannen. In totaal zijn er 437 zedendelinquenten opgenomen in deze studie.
De resultaten tonen aan dat, over het algemeen, deze instrumenten een matige tot hoge voorspellende betrouwbaarheid hebben wat betreft het optreden van recidive, als ook wat betreft het aantal recidieven. De Static-2002R en Static-99R kwamen als beste uit de test. De Sexual Violence 20 en Rapid Risk Assessment for Sexuel Offense Recidivism lieten de minst positieve resultaten zien, respectievelijk voor seksuele recidivisme en agressieve recidivisme.
De resultaten wijzen op een positief verband tussen de voorspellende accuraatheid van de instrumenten en de de tussen-beoordelaars-betrouwbaarheid op itemniveau, zowel wat betreft seksuele recidive (correlatie: r = .28, p = .01) als ook wat betreft gewelddadige recidive (r = .45, p < .001).
In deze studie is er geen verband aangetoond tussen de voorspellende accuraatheid van de instrumenten en missende waardes op itemniveau. Dit betekent dat de instrumenten, ook wanneer er missende data zijn, een goede voorspellende waarde hebben.
Samengevat benadrukken deze resultaten de geschiktheid en het nut van deze instrumenten voor het selecteren van behandeling volgens de risico-behoefte-responsiviteit principes. Hoewel alle negen instrumenten betere resultaten laten zien dan op basis van toevallig (willekeur), zijn de Static-2002R en Static-99R het meest consistent over verschillende uitkomstmaten en follow-up perioden.
De auteurs sluiten af met een kritische noot omtrent het Nederlandse beleid betreffende de behandeling van zedendelinquentie. Specifieker: zij beweren dat deze resultaten benadrukken dat het belangrijk is om het beleid in Nederland aan te passen: risicobeoordeling instrumenten moeten niet alleen gebruikt worden als "een stapje" in een groter, geïndividualiseerd besluitvormingsproces, maar zouden de basis moeten vormen voor de selectie en het beloop van de behandeling van zedendelinquenten.
