Anamnese bij urinesteenlijden

De anamnese bij urinesteenlijden bestaat uit het uitvragen van de pijn qua aard, bewegingsdrang, lokalisatie en uitstraling, maar ook een acuut begin, misselijkheid en braken, bloed in de urine. Verder vragen die gesteld kunnen worden zijn eerder urinesteenlijden in de voorgeschiedenis, familiair voorkomen van urinestenen, aangeboren afwijkingen, een nierfunctiestoornis en mictieklachten en koorts.

Lichamelijk onderzoek

Tijdens het lichamelijk onderzoek kan de aanwezigheid van bewegingsdrang worden opgemerkt, de temperatuur en bloeddruk kunnen gemeten worden en er kan onderzoek van de buik plaatsvinden. Wanneer druk- of slagpijn in de nierloge aanwezig is, is sprake van een urinesteen. Wanneer peritoneale prikkeling aanwezig is, past dit niet bij een ongecompliceerd urinesteenlijden.

Aanvullend onderzoek

Het aanvullend onderzoek bij een verdenking op urinesteenlijden bestaat uit een urineonderzoek aan de hand van een dipstick. Er kunnen erytrocyten en nitriet worden aangetoond.

Beleid

Het beleid bij urinesteenlijden bestaat uit voorlichting en advies over normaal blijven eten en drinken, opvangen van het steentje. Verder moet de patiënt opnieuw contact opnemen wanneer koorts, mictieklachten en oncontroleerbare pijn voorkomen. Er moet na vijf tot zeven dagen gecontroleerd worden. In eerste instantie wordt pijnstilling in de vorm van diclofenac toegediend. Verder kan tamsulosine voorgeschreven worden om de steenlozing versneld te laten plaatsvinden. Echter kan dit wel gepaard gaan met bijwerkingen.

  3. Tags & Taxonomy: gives you insight in the amount of summaries that are tagged by authors on specific subjects. This type of navigation can help find summaries that you could have missed when just using the search tools. Tags are organised per field of study and per study institution. Note: not all content is tagged thoroughly, so when this approach doesn't give the results you were looking for, please check the search tool as back up
