Anamnese bij mictieklachten bij mannen

Tijdens de anamnese moet gevraagd worden naar het moeilijk op gang komen van de mictie, een zwakke of onderbroken straal, minder goed uitplassen, een moeilijk te bedwingen aandrang, nadruppelen, toegenomen mictiefrequentie overdag en (nachtelijke) incontinentie. Verder moet gevraagd worden naar pijn bij het plassen, of pijn in het perineum, een branderig gevoel of afscheiding, hematurie, koorts, koude rillingen, algemene malaise, flankpijn en urineweginfecties in de voorgeschiedenis. Tenslotte moet nog gevraagd worden naar de snelheid van ontstaan of verergering van de klachten, of de aanwezigheid van obstipatie. Verder moet gevraagd worden naar het medicatiegebruik, de voorgeschiedenis en aanwezige comorbiditeit. Snel ontstane klachten in combinatie met perianale koorts en/of koude rillingen kan duiden op acute prostatitis. Een urineweginfectie wordt gekenmerkt door mictieklachten, een positieve nitriettest en dipslide en kweek met afwijkend sediment. Een urethrastrictuur wordt gekenmerkt door mictieklachten bij een lokaal trauma, urologische ingreep of uretritis in de voorgeschiedenis. Een prostaat met asymmetrische vorm, onregelmatige consistentie of harde nodus past bij prostaatcarcinoom. Een acute urineretentie tenslotte wordt gekenmerkt door het onvermogen tot spontaan plassen, ondanks aandrang en meerdere pogingen in korte tijd, gedempte percussie en een (pijnlijke) gevulde blaas.

Lichamelijk onderzoek

Het lichamelijk onderzoek bestaat uit inspectie van de onderbuik, palpatie en percussie van de blaasregio en een rectaal toucher.

De urine kan onderzocht worden op tekenen van infectie.

Behandeling

De behandeling bij mictieklachten bij mannen bestaat uit voorlichting geven over het vaak voorkomen van aspecifieke mictieklachten, een vergroting van de prostaat niet per se hoeft te betekenen dat dit gerelateerd is aan mictieklachten en ook moet het beloop van de mictieklachten besproken worden. Wanneer sprake is van incontinentie, moet bekkenbodemfysiotherapie overwogen worden en bij nycturie moet de vochtinname ’s avonds worden beperkt. Wanneer de klachten aspecifiek zijn, kan uitleg gegeven worden over het belang van voldoende lichaamsbeweging, een verminderde vochtinname en bekkenbodemfysiotherapie. Wanneer hinderlijke urge-incontinentie plaatsvindt, kan een anticholinergicum toegediend worden en wanneer hinderlijke aspecifieke klachten aanwezig zijn, kan alfuzosine, of tamsulosine toegediend worden.
