NHG-standaard (Geneeskunde): Acuut coronair syndroom - M80
Het acuut coronair syndroom bestaat uit een acuut myocardinfarct (AMI), of een instabiele angina pectoris (IAP) met klachten zowel tijdens inspanning als in rust.
Anamnese
De anamnese bij verdenking op acuut coronair syndroom bestaat uit het uitvragen van de duur van de klachten in rust, de locatie van de pijn, bijkomende vegetatieve verschijnselen, dypsneu, hartziekten in de voorgeschiedenis, de aard van de pijn. Verder moet het medicatiegebruik uitgevraagd worden, bekende allergieën, comorbiditeit en intoxicaties. Duizeligheid, collaps en bewustzijnsverlies hoort bij circulatoire problemen, terwijl kortademigheid en niet plat kunnen liggen bij acute decompensatie past.
Lichamelijk onderzoek
Het lichamelijk onderzoek bij verdenking op acuut coronair syndroom bestaat uit het beoordelen van de klinische stabiliteit van de patiënt volgens de ABCDE-methode. De pols en bloeddruk moeten gemeten worden, het hart en de longen moet geausculteerd worden en er moet palpatie van de thoraxwand plaatsvinden als de patiënt pijn aangeeft.
Behandeling
De medicamenteuze behandeling van een acuut coronair syndroom bestaat uit nitroglycerinespray sublinguaal bij pijn, maar niet als de systolische bloeddruk lager is dan 90 mm Hg. Er kunnen maximaal drie doses worden gegeven gedurende iedere vijf minuten. Zuurstof moet toegediend worden bij respiratoir falen via een non-rebreathing masker. Wanneer de saturatie weer hoger is dan 94% kan een zuurstofbril zorgen voor extra zuurstoftoediening. Een waaknaald moet ingebracht worden met een NaCl oplossing. Morfine of fentanyl kan gegeven worden bij hevige pijn en onvoldoende reactie op nitraten. Ascal moet toegediend worden bij patiënten die nog niet bekend zijn met het gebruik van Ascal. Als er sprake is van bradycardie moet atropine intraveneus worden toegediend. Tenslotte moet er gezorgd worden voor een goede overdracht naar het ambulancepersoneel.
Follow-up
Wanneer herstel heeft plaatsgevonden moet het medicatiegebruik regelmatig gecontroleerd worden en er moet gevraagd worden naar ervaringen en mogelijk aanwezige angsten van de patiënt. Verder moet hartrevalidatie gestimuleerd worden. Verder moeten de klachten, leefstijl en medicatiegebruik verder uitgevraagd worden. Maagbescherming moet eventueel voorgeschreven worden. De pols moet gepalpeerd worden, de bloeddruk en het lichaamsgewicht moet gemeten worden, bloedonderzoek moet uitgevoerd worden en er moet voorlichting gegeven worden over een eventuele nieuwe episode met acuut coronair syndroom.
