Zelfrapportage vragenlijst kan je nooit gebruiken voor een diagnose.
Tegenwoordig is de consensus:
- The Happiness Trap: als ik me niet gelukkig voel, dan gaat het slecht. Nee, je emoties komen en gaan. Juist door dat te accepteren, dat je niet altijd gelukkig bent, word je gelukkiger.
Relatie tussen persoonskenmerken en symptomen = uitingsvorm.
- Er zit een positieve kant aan ieder symptoom. Maar daardoor blijft het ook in stand gehouden.
- Controledrang kan ook leiden tot gestructureerd persoon, mogel beter zijn best doen op school.
- Wat wint deze persoon met het instandhouden van zijn coping?
DSM-V
- Die labels uit de DSM-V zijn een beetje hun eigen leven gaan leiden.
- Geen enkele stroming verklaart het helemaal. Wees niet te kritisch op elke stroming, maar kies de goede punten uit elke stroming.
- Als het geen classificatie heeft, weten we geen behandeling erbij.
Leer de prevalentie van stoornissen!
DSM limitaties
- Comorbiditeit is de regel
- Afbakening onduidelijk want veel overlap
- Versnippering, we hebben nu zo’n 500 diagnoses
- Puur gericht op symptomen en niet onderliggende etiologie (oorzaak)
Drie andere manieren om te classificeren.
Research Domain Criteria (RDOC):
Symptoomclusters linken aan neurobiologie
Hierarchical taxonomy of psychopathology
- Psychopathologie spectra
- Ze wilden af van de hokjes
- LEER DIT GOED: externaliserende vs internaliserende problematiek
Netwerktheorie
- Kijkt alleen nog maar naar de losse symptomen
- Hoe hangen die symptomen samen
- Geen achterliggend construct.
Rdoc is gebaseerd op psychofarmaca. Niet op therapeutisch gebied. Alle psychiatrische stoornissen zouden een biologische oorzaak hebben. Hitop is ieder symptoom op een spectrum, soort DSM.
RDOC limitaties:
- Corticolimisch systeem is aangedaan bij alle psychiatrische stoornissen.
- Neurobiologische constructen zijn weinig specifiek
- Neurobiologische constructen verklaren maar een klein deel
- Zelfs als het zou werken: meeste klinische praktijken hebben niet de juiste apparatuur (MRI-scans enzo)
- Dit zijn wel goede bouwblokken voor verder onderzoek naar onderliggend neurobiologie
HITOP limitaties:
- Combinaties van uitersten op een spectrum niet mogelijk
- General factor, is nog niet bekend wat dat inhoudt
- Wel goed overzicht van samenhang tussen stoornissen
Netwerkmodel limitaties:
- Het gaat puur om correlaties tussen symptomen die gecorrigeerd zijn voor andere onderlinge correlaties
- GEEN CAUSALITEIT
- Wel inzichtgevend: samenhang, overlap en individuele aangrijpingspunten
De diagnose
- Beschrijvende diagnose: hoofd van bijzaken scheiden.
- Subjectief lijden en disfunctie zijn de belangrijkste criteria.
- Iemand die raar gedrag vertoont? Maar hij lijdt niet en is zijn omgeving niet tot last, dan is er niks aan de hand.
