NHG-standaard (Geneeskunde): Vaginaal bloedverlies - M28
Anamnese
Wanneer bloedverlies vaginaal plaatsvindt in de reproductieve fase moet gevraagd worden naar de aard van het bloedverlies en de ervaren hinder ervan. Ook moeten aanwijzingen voor specifieke oorzaken worden uitgevraagd, zoals een zwangerschap, anticonceptie, pijn in de onderbuik of toename menstruatiepijn, risico op een SOA, geneesmiddelengebruik, opvliegers, ontstaan van klachten na sectio en snel optreden van blauwe plekken, wondjes en stollingsafwijkingen. Wanneer vaginaal bloedverlies plaatsvindt na de reproductieve fase moet het begin, beloop en de duur uitgevraagd worden. Ook moet gevraagd worden naar het risico op een SOA, het geneesmiddelengebruik en naar post coïtaal bloedverlies.
Lichamelijk onderzoek
Het lichamelijk onderzoek moet alleen uitgevoerd worden bij iemand met een verhoogd risico op een SOA, of bij postmenopauzaal bloedverlies. Er moet inspectie van de vulva en het perineum plaatsvinden, evenals speculumonderzoek en een vaginaal toucher.
Aanvullend onderzoek
Er moet een zwangerschapstest uitgevoerd worden indien het risico hierop bestaat, een chlamydiatest bij het risico op een SOA, de temperatuur moet gemeten worden. Verder kan een cervixuitstrijk uitsluitsel geven over contactbloedingen en zichtbare anatomische afwijkingen. Verder kan een menstruatiekalender bijgehouden worden en een menstruatiescorekaart. Het Hb kan bepaald worden, maar ook het stollingsonderzoek en een transvaginale echoscopie kunnen plaatsvinden. Wanneer iemand postmenopauzaal bloedverlies heeft, moet een cervixuitstrijk gemaakt worden, een transvaginale echoscopie en chlamydiatest.
Behandeling
De behandeling bestaat uit uitleg geven over de aandoening. Verder kunnen NSAIDs genomen worden, tranexaminezuur, de combinatiepil, een hormoonspiraal en oraal progestageen gedurende vijf tot tien dagen.
Follow-up
De follow-up van vaginaal bloedverlies bij medicatiegebruik bestaat uit na drie tot zes maanden terugkomst. Verder wanneer de medicatie onvoldoende helpt, of het probleem aan blijft houden moet opnieuw een transvaginale echoscopie en cervixuitstrijk gemaakt worden.
