SOLK staat voor Somatisch Onvoldoende verklaarde Lichamelijke Klachten. Men spreekt van SOLK wanneer er al enige tijd langdurige lichamelijke klachten aanwezig zijn en als er na uitgebreid onderzoek geen oorzaak van de klachten wordt gevonden. Tijdens de anamnese worden de uitgebreid de klachten uitgevraagd, moet de patiënt vooral zijn verhaal kunnen doen en worden alle dimensies van de klachten middels het de zogenaamde SCEGS model uitgevraagd. SCEGS staat voor; Somatisch-, Cognitieve-, Emotionele-, Gedragsmatige- en Sociale dimensie. Verricht tevens lichamelijk onderzoeken eventueel aanvullend onderzoek. Maak vervolgens een keuze voor het type aanvullend onderzoek (middels NHG standaarden) en bespreek de reden van het onderzoek dat je uitvoert. Vervolgens moet de ernst van de SOLK worden beoordeeld. Dit wordt gedaan aan de hand van de klachtexploratie, de mate van belemmering voor de patiënt, het aantal klachten en de duur van de klachten. Vervolgens wordt SOLK ingedeeld in milde-, matige- en ernstige SOLK.
Onder milde SOLK wordt het volgende verstaan:
- lichte functionele belemmering en daarnaast één of een paar SOLK klachten binnen één of twee klachtenclusters.
- wordt door de huisarts vastgesteld
Met matige SOLK wordt het volgende omschreven:
- matige belemmeringen
- meerdere SOLK klachten in ten minste drie klachtenclusters
- de duur van de klachten is langer dan verwacht
- wordt in samenwerking met andere eerstelijnshulpverleners gesteld
- samenwerking met fysiotherapeut, POH-ggz (praktijkondersteuner) of cognitief-gedragsmatige geschoolde ELP (eerstelijnspsycholoog)
Met ernstige SOLK wordt het volgende bedoeld:
- ernstige belemmeringen
- SOLK klachten in alle klachtenclusters
- de klachtenduur is langer dan drie maanden
- wordt vastgesteld in samenwerking met tweedelijnshulpverleners
- vaak wordt er samengewerkt of verwezen naar een multidisciplinair team of behandelcentra
Wanneer er naast de SOLK ook sprake is van een depressieve- of angststoornis, wordt de dubbeldiagnose besteld. Dit in het geval dat beide klachten ernstig zijn, waarbij ze beide een aparte behandeling nodig hebben.
