Er is tegenwoordig een opkomst van evidence-based medicine. Wanneer er genoeg bewijs bestaat voor een bepaalde behandeling wordt deze vaak uitgevoerd. Echter is de inbreng van de individuele patiënt ook van belang bij de uiteindelijke beslissing over de behandeling. De behandelaar beslist in samenspraak met de patiënt of er behandeld wordt volgens de richtlijn, of dat er van de richtlijn afgeweken wordt. Zorgverzekeraars bieden pakketten waar vraag naar is en bevatten dus veel behandeling waarvan geen bewijs is dat ze effectief zijn. Richtlijnen kunnen ondersteuning bieden aan de arts en ook de patiënt weet dat hierdoor de behandeling bij verschillende artsen nagenoeg hetzelfde zal zijn. Randomized controlled trials (RCT) kunnen een bijdrage leveren aan het leveren van wetenschappelijk bewijs voor een bepaalde behandeling. Echter kunnen er ook obstakels zijn om deze onderzoeken uit te voeren, zoals methodologische obstakels, financiële obstakels of ethische obstakels. Het gevaar van evidence-based medicine is dat niet-bewezen interventies steeds meer onder druk komen te staan. Bovendien moet ervoor gewaakt worden dat mensen in hun zoektocht naar bewijs niet vergeten om aandacht te hebben voor de patiënt. De zorgverzekeringen bieden evidence-based zorg aan in het basispakket, terwijl daarbij het perspectief van de patiënt niet meer meegenomen wordt. De wens van de patiënt is in het aanvullend pakket nog wel doorslaggevend. De kwaliteit van de zorg komt onder druk te staan, doordat er minder ruimte is voor de voorkeur van de patiënt en het oordeel van de behandelaar. Bovendien kan ook bewezen worden dat bepaalde behandelingen niet werken. Het is dus belangrijk dat er een minder groot gat komt tussen het basispakket en het aanvullend pakket van de zorgverzekering. De evidence-based medicine heeft ervoor gezorgd dat de behandelend arts niet meer zijn keuzes op zijn eigen kennis baseert, maar de richtlijnen meeneemt in zijn beslissingen. Het belang van de patiënt moet echter nog steeds de belangrijkste reden zijn om een bepaalde behandeling te starten of niet. Het aanvullend pakket van de zorgverzekering houdt meer rekening met de eigen keuzen van de patiënt, terwijl in het basispakket vooral de solidariteit centraal staat.
