Het plaveiselcelcarcinoom - Marks - Artikel
Basaalcelcarcinoom is de meest voorkomende huidkanker, gevolgd door plaveiselcelcarcinoom. Risicofactoren die bij kunnen dragen aan de ontwikkeling van plaveiselcelcarcinoom zijn het lichtere huidtype en onbeschermde blootstelling aan de zon. De gemiddelde leeftijd waarop plaveiselcelcarcinoom gediagnosticeerd wordt is 60 tot 65 jaar. De blootstelling aan met name UV-B en deels UV-A leveren een bijdrage aan de ontwikkeling van plaveiselcelcarcinoom. Mannen hebben een groter kans op het ontwikkelen van plaveiselcelcarcinoom, evenals mensen die veel buiten werken of hebben gewerkt. Uv-straling zorgt voor DNA schade in de cellen van de RAS en tumorsuppressorgenen. Ook wanneer iemand behandeld is met immunosuppresiva is de kans hoger op plaveiselcelcarcinoom. Ook mensen die besmet zijn geweest met HPV-virus hebben een verhoogde kans op het krijgen van plaveiselcelcarcinoom. De tumor groeit snel en kan variëren in grootte. De tumoren raken ontstoken en kunnen bloeden. De voorkeurslocaties zijn het hoofd en de nek en verder de aan zon blootgestelde huid. Wanneer de tumor metastaseert vindt dit plaats in de lymfeknopen, gevolgd door de longen, lever en andere organen. Histologisch onderzoek kan de diagnose van plaveiselcelcarcinoom bevestigen, eventueel aangevuld met een biopsie.
De behandeling bestaat in principe uit excisie, maar wanneer de tumor het functioneren van de organen beperkt moet radiotherapie een oplossing bieden, net als Mohs chirurgische ingreep. Radiotherapie wordt met name gegeven aan mensen met metastases. Curettage en diathermie, cyrochirurgie en biologicals worden ook gebruikt voor de behandeling van plaveiselcelcarcinoom. De follow-up na behandeling is erg belangrijk in verband met de mogelijke ontwikkeling van metastases. De meeste ontwikkelen zich twee jaar na de presentatie van het ziektebeeld en uiterlijk vijf jaar na de presentatie van het ziektebeeld. Het eerste jaar bestaat de follow-up uit eens per drie maanden en later eens per half jaar. Wanneer een groot risico bestaat op metastases moet de follow-up frequenter plaatsvinden. Er wordt gedacht dat bescherming tegen de zon met zonnebrandcrème de ontwikkeling van plaveiselcelcarcinoom tegen zou moeten gaan.
