Zwangerschap
Er zijn geen bewezen risico’s van benzodiazepines op zwangerschaps- en baringcomplicaties. Daarom kan een goed ingestelde benzodiazepine gewoon voortgezet worden tijdens de zwangerschap en zijn er geen extra controles nodig. Als er sprake is van een hoge dosering, chronisch gebruik of gebruik van andere psychofarmaca moet hier wel extra naar gekeken worden.
Benzodiazepines hebben geen verband met verhoogde kans op aangeboren afwijkingen. Wel is er kans op neonatale adaptatie problemen bij hoge dosering, chronisch gebruik, comedicatie en vroeggeboorte. Ouders dienen hiervan vooraf op de hoogte besteld te worden. Maar Structureel echoscopisch onderzoek (SEO) volstaat bij chronisch of incidenteel benzodiazepinegebruik in de zwangerschap.
De geboorte hoeft niet in een eerstelijns geboortecentrum of ziekenhuis plaats te vinden. Tenzij in het derde trimester van de zwangerschap intermitterend benzodiazepinegebruik is, dan wordt namelijk aangeraden om onder verantwoordelijkheid van de eerstelijns verloskundige of verloskundig huisarts te bevallen vanwege het toegenomen risico op een lage Apgarscore. Zolang het kind een goede conditie heeft en de ouders goed worden geïnstrueerd over het kraamzorg en het mogelijk optreden van neonatale adaptatiestoornissen, is klinische observatie van de pasgeborene niet nodig. Dit geldt niet voor moeders die in het derde trimester nog benzodiazepines gebruiken, hierbij is klinische observatie wel nodig.
Borstvoeding
Als benzodiazepines incidenteel of eenmalig worden gebruikt is er geen indicatie om te borstvoeding te onderbreken. Bij chronisch gebruik moet men hier voorzichtiger mee zijn. Dit hangt af van de dosering, halfwaardetijd en uitscheiding in de borstvoeding. Bij hoge dosering of comedicatie wordt het geven van borstvoeding ontraden.
Staken van benzodiazepines
Als men ervoor kiest te stoppen met de medicatie tijdens zwangerschap of lactatie moet rekening gehouden worden met het risico op terugval van de psychiatrische aandoening. Hierbij zijn de volgende factoren van belang: de indicatie voor het benzodiazepinegebruik, de duur van het gebruik, eerdere stoppogingen, de mogelijkheden van ondersteuning tijdens het afbouwen en beschikbaarheid van andere behandelingsopties.
