Een juiste bepaling van de zwangerschapsduur reduceert het aantal inleidingen in verband met serotiniteit en kan helpen bij de reductie van perinatale morbiditeit en mortaliteit doordat untra-uteriene groeirestrictie of macrosomie snel kunnen worden opgespoord. In het eerste trimester kan een test gedaan op worden op aanwezigheid van het Down syndroom bij de foetus, een test voor de nekplooidikte, de maternale leeftijd en maternale serummarkers, zoals HCG. De zwangerschapsduur kan berekend worden aan de hand van de zogenaamde LM, oftewel de laatste normale menstruatie, maar het kan ook berekend worden aan de hand van de echoscopische meting van de kop-romp lengte (CRL), de distantia biparietalis (DBP) of de hoofdomtrek (HC).
Een eenduidige zwangerschapsdatering is essentieel voor de samenwerking tussen de eerstelijnszorg en tweedelijnszorg, maar ook met de perinatologische centra. De kop-romp lengte wordt gemeten in het mid-sagitale vlak met het caput, een extremiteit of het genitale tubercle en de wervelkolom en is de afstand van de kruin tot de stuit. De distantia biparietalis wordt gemeten van de buitengrens tot de andere buitengrens van de schedel die loodrecht op de middellijn staat. De hoofdomtrek wordt gemeten door een elips om de schedel te projecteren in het transversale vlak zonder daarbij de huid en subcutis mee te nemen. De occipitale frontale diameter (OFD) staat loodrecht op de DBP van craniaal naar caudaal. De transcerebellaire diameter wordt gemeten aan de hand van een oblique transversale doorsnede door het caput van de foetus.
Aanbevelingen voor het meten van de zwangerschapsduur
De zwangerschapsdata worden bepaald aan de hand van een echoscopisch onderzoek met de HC, TCD en CRL. De CRL kan berekend worden vanaf acht weken en twaalf weken en de optimale zwangerschapsdatering wordt verkregen tussen week tien en week twaalf van de zwangerschap. De echo moet transvaginaal plaatsvinden wanneer de zwangerschap nog niet langer is dan tien weken en wordt nooit een termijnecho genoemd. Wanneer meerdere metingen worden gedaan tussen de tien en twaalf weken, wordt de meting die het dichtst bij de tien weken ligt, genomen als datering van de zwangerschap. De DBP en HC kunnen het beste ook gemeten worden vanaf de dertiende week van de amenorroe. Wanneer blijkt dat de HC-waarde groter is dan 80 millimeter, wordt de zwangerschap op basis daarvan gedateerd. De TCD kan het beste gemeten worden vanaf de achttiende week van de zwangerschap.
