Hoorcollege 11: somatoforme stoornissen (Psychopathologie 2, UU)
Onbewust nabootsen komt vaker voor dan bewust (malingering). Somatoforme stoornissen komen veel voor.
Predisponerende factoren:
- Biologische gevoeligheid
- Traumatische jeugd
- Persoonlijkheidstrekken
Uitlokkende factoren:
- Infectie
- Letsel
- Stresserende gebeurtenissen
Er zijn best wel veel verklaringsmodellen:
- Bio-psychosociale model: de context is belangrijk
- Draagkracht: er is disbalans in de draagkracht en draaglast
- Stress model: stress resulteert in onverklaarbare klachten
- Perceptueel-cognitief: cognitieve aandacht wordt gericht op lichamelijke sensaties
- Neurobiologische modellen: disbalans in HPA as etc.
- Centrale sensitisatie: pijnperceptie in brein is verstoord
- Vicieuze cirkels
- Placebo / nocebo: klassieke conditionering
- Individuele ontwikkeling: hechtingsstijlen, affectieve verwaarlozing
- Religieuze verklaringsmodellen
Diagnostisch stappenplan:
- Anamnese en lichamelijk onderzoek
- Psychiatrische stoornissen
- In stand houdende en prognostisch ongunstige factoren
Verwijzing tweede lijn etc.
In de tweedelijn wordt er gekeken naar middelengebruik, nagebootste stoornis, etc. Kan ook in combinatie met een lichamelijke aandoening.
Ziekte is maar 1 oorzaak van symptomen, kan ook sociale/culturele factoren, fysiologische processen, etc.
1/3 van de neurologie patiënten op de poli heeft ‘functionele’ klachten. Worden niet helemaal of helemaal niet verklaard door neurologische ziekte.
Gevolgenmodel (CGT):
- Je doet geen uitspraak over de oorzaak, maar de gevolgen staan centraal
- De gevolgen kunnen de aandoening in stand houden of verergeren
- Focus ligt op optimaliseren van de omstandigheden voor herstel van de klachten
- Diagnostisch proces waaruit handvatten voor behandeling voortkomen
- Draait nu meer om het optimaliseren van de omstandigheden voor herstel in plaats van verminderen. Van de hinderlijke gevolgen
In de praktijk vraag je ten eerste naar de lichamelijke klachten. Ook vraag je naar somatische geschiedenis. Daarna ga je de overtuigingen bespreken (ideeën over de betekenis van de klacht). Ook de inhoud van de cognities (catastroferend, niet-accepterend, zelfdeprimerend) zijn belangrijk. Daarna: emotionele en gedragsmatige gevolgen. Ook de lichamelijke gevolgen (observatie, conditie, etc.) en sociale gevolgen.
Sociale gevolgen zijn zelden een rechtstreeks instandhoudende factor. Behandel eerst de lichamelijke, gedragsmatige en emotionele gevolgen.
