Een acute stressstoornis (ASS) is een psychische aandoening die kan ontstaan direct na het meemaken van een ernstige traumatische gebeurtenis. Dit kan een ongeluk, ramp, geweldsmisdrijf, terroristische aanslag of andere ingrijpende gebeurtenis zijn. Mensen met een ASS ervaren intense angst, herbelevingen van de gebeurtenis en andere psychische en lichamelijke symptomen.
Wat zijn de belangrijkste symptomen van een acute stressstoornis?
- Herbelevingen: De traumatische gebeurtenis wordt steeds opnieuw in gedachten of dromen herbeleefd.
- Vermijding: De neiging om plekken, mensen of activiteiten te vermijden die gerelateerd zijn aan de traumatische gebeurtenis.
- Negatieve cognities: Negatieve gedachten en overtuigingen over zichzelf en de wereld, zoals "Ik ben waardeloos" of "De wereld is een gevaarlijke plek."
- Verhoogde prikkelbaarheid: Snel schrikken, geïrriteerd zijn, moeite hebben met concentreren.
- Slaapproblemen: Moeilijk in slaap vallen of doorslapen, nachtmerries.
- Lichamelijke klachten: Hartkloppingen, zweten, trillen, hoofdpijn, buikpijn.
Waarom is een acute stressstoornis belangrijk?
Een ASS kan een ernstige impact hebben op het leven van een persoon. De symptomen kunnen leiden tot verminderd functioneren op het werk, school of in sociale relaties. Mensen met een ASS hebben ook een verhoogd risico op het ontwikkelen van andere psychische aandoeningen, zoals posttraumatische stressstoornis (PTSS).
Prevalentie in Nederland
Ongeveer 1% van de mensen die een traumatische gebeurtenis meemaken, ontwikkelt een ASS. De aandoening komt vaker voor bij vrouwen dan bij mannen.
Hoe werkt een acute stressstoornis in de praktijk?
De symptomen van een ASS ontstaan meestal binnen de 72 uur na de traumatische gebeurtenis. De symptomen kunnen acuut zijn en kortdurend (meestal enkele dagen tot weken), of verlengd (enkele weken tot maanden). In sommige gevallen kunnen de symptomen overgaan in PTSS.
Praktijkvoorbeeld
Een vrouw is betrokken bij een ernstig auto-ongeluk. Na het ongeluk heeft ze last van flashbacks van het ongeluk, vermijdt ze drukke plekken en heeft ze moeite om te slapen. Ze is ook prikkelbaar en heeft last van concentratieproblemen. De vrouw heeft een ASS.
Wat is de behandeling van een acute stressstoornis?
De behandeling van ASS richt zich op het verminderen van de symptomen en het bevorderen van het herstel. De meeste mensen met ASS herstellen binnen enkele weken of maanden zonder behandeling. Echter, voor mensen met ernstige symptomen of die moeite hebben om te herstellen, kunnen verschillende behandelingen nuttig zijn:
1. Psychotherapie:
- Cognitieve gedragstherapie (CGT): CGT is de meest effectieve therapie bij ASS. Het helpt mensen met ASS om:
- Negatieve gedachten en overtuigingen te herkennen en te veranderen.
- Gezonde coping-mechanismen te ontwikkelen om met de symptomen om te gaan.
- De traumatische gebeurtenis te verwerken.
- Eye Movement Desensitization and Reprocessing (EMDR): EMDR is een therapie die gericht is op het verwerken van traumatische herinneringen. Tijdens EMDR worden oogbewegingen gecombineerd met het herbeleven van de traumatische gebeurtenis.
- Andere vormen van therapie: Stressmanagement technieken, ontspanningstherapie, en psychodynamische therapie kunnen ook nuttig zijn.
2. Medicatie:
In sommige gevallen kan medicatie worden voorgeschreven om symptomen zoals angst, depressie, slaapproblemen of nachtmerries te verminderen. Vaak gebruikte medicijnen zijn:
- Antidepressiva: Deze medicijnen kunnen helpen om symptomen van angst en depressie te verminderen.
- Benzodiazepines: Deze medicijnen kunnen helpen om angst en slaapproblemen te verminderen. Het is echter belangrijk om deze medicijnen met voorzichtigheid te gebruiken vanwege het risico op afhankelijkheid.
- Bètablokkers: Deze medicijnen kunnen symptomen zoals hartkloppingen, trillen en zweten verminderen.
3. Ondersteunende zorg:
Naast psychotherapie en medicatie kan ondersteunende zorg ook een belangrijk onderdeel van de behandeling zijn. Dit kan bestaan uit:
- Psycho-educatie: Leren over ASS, de symptomen en de behandeling.
- Lotgenotencontact: Deelnemen aan een steungroep voor mensen met ASS kan helpen om ervaringen te delen en van elkaar te leren.
- Praktische hulp: Hulp bij het regelen van dagelijkse zaken, zoals financiën of huisvesting.
Aandachtspunten
- ASS is een tijdelijke aandoening. De meeste mensen met een ASS herstellen binnen enkele weken of maanden.
- Het is belangrijk om professionele hulp te zoeken als je na een traumatische gebeurtenis last hebt van de symptomen van ASS.
- Er is geen wondermiddel voor ASS. Behandeling vereist tijd en inspanning.
- Goede sociale steun van familie en vrienden kan een belangrijk onderdeel van het herstelproces zijn.
