Sinds het bestaan van een ultra high risk (UHR) criterium voor psychoses, is het doel om preventief in te grijpen bij UHR mensen met een eerste episode van psychoses.Meta-analyse heeft aangetoond dat CBT geassocieerd is met een gehele risicovermindering bij mensen met een psychose. Ander onderzoek uit Nederland heeft aangetoond dat CBTUHR een psychose kan uitstellen of zelfs voorkomen en dat CBT gebruikt moet worden als bewijsmatige behandeling bij UHR symptomen.
Deze studie onderzoekt wanneer een eerste psychotische episode geassocieerd wordt met een slechtere klinische en sociale uitkomst in vergelijking met de groep die de drempelwaarde criteria niet bereiken. Ising maakt gebruik van een 4-jarige follow up data van nederlandse EDIE-NL trials waarbij het volgende geevalueerd wordt:
- Of CBTuhr preventieve effecten blijft hebben voor eerste episode psychoses (significant)
- Of CBTuhr remissie bevordert bij een UHR status (significant)
- De klinische en functionele status bij degenen met een psychotische status (niet significant)
De huidige studie toont aan dat het preventieve CBTuhr-interventie succesvol is in het halveren van de 4-jaars cumulatieve incidentie van psychose bij UHR-deelnemers in vergelijking met TAU (Treatment as usual).
Een psychose kan voorkomen worden bij elke 8 patiënten met UHR die CBT krijgen in vergelijking met TAU. Vervolgstudies moeten zich focussen op bredere uitkomsten van de behandeling, omdat UHR niet alleen in relatie staat met psychoses maar een bredere psychopathologie heeft.
Deze studie heeft een aantal limitaties. 42 procent heeft de vierjarige follow up niet gehaald. Dit zou kunnen liggen aan een verbetering in gezondheidsproblemen bij de participanten of terug vielen in een psychose. Dit maakt de uitkomsten van het onderzoek minder inzichtelijk, omdat deze patiënten die uitvallen, juist essentieel waren voor de statistieken. Daarnaast hebben de participanten die terugvielen in een psychose, de eerste 18 maanden niet dezelfde follow up gehad als degenen die niet terugvielen in een psychose. Sommige metingen konden als gevolg niet juist worden omgezet.
Ondanks de opgesomde negen limitaties van het onderzoek, kan geconcludeerd worden dat de prognose aanzienlijk verbetert voor patienten met UHR symptomen bij de CBTuhr interventie en dat deze verbetering zorgt voor het voorkomen van eerste psychotische episodes. Er is na deze behandeling nog steeds sprake van een affect op gebied van sociaal functioneren bij de meeste participanten. Daarnaast hebben participanten nog steeds last van positieve, negatieve en cognitieve symptomen.
