BPOP Hoorcollege 1 - Psychotische stoornissen
Psychose: werkelijkheid en fantasie zijn niet te onderscheiden; nieuwe realiteit. Betreft meestal wanen (irrationeel geloof (geloof van iets wat er niet is)) en/of hallucinaties (sensorische ervaring in afwezigheid van externe gebeurtenis).
Vrijwel iedereen die schizofrenie heeft, heeft een psychose doorgemaakt. Maar niet iedereen die een psychose heeft doorgemaakt heeft schizofrenie.
DSM-5 criteria schizofrenie:
- Twee of meer van het volgende: wanen, hallucinaties, gedesorganiseerd spreken / gedrag, negatieve symptomen
- Na de diagnose wordt het functioneren lager
- Zes maanden aanwezig
In DSM-5 zijn de subtypes vervallen en de ernst van de symptomen zijn in kaart gebracht op een 4-puntsschaal.
Bij veel psychopathologie is er sprake van een verkleinde hippocampus.
fMRI onderzoek bij mensen tijdens het horen van stemmen:
- Activiteit in taal perceptie en productie gebieden
- Activiteit in de primair gehoor-schors
- Activiteit vooral in de rechter hersenhelft
- Lijkt getriggerd vanuit het geheugen
>50% van de stemmen negatief: 82% kans op een psychotische stoornis
Alle stemmen negatief: 90% kans op psychotische stoornis
Diathese stress model: bij een grotere kwetsbaarheid is er minder stress en bij een kleinere kwetsbaarheid meer stress nodig is om de stoornis aanvang te laten nemen of voor verergering van de symptomen te zorgen.
Risicofactoren schizofrenie:
- Plek van opgroei
- Cannabis gebruik
- Kip-ei probleem (zorgt cannabisgebruik voor schizofrenie of schizofrenie voor cannabisgebruik?). Hier is veel onduidelijkheid over maar de laatste onderzoeken laten zien dat cannabis weldegelijk een causale rol heeft bij het ontwikkelen van een psychotische stoornis
- Migratie
- Sociaal economische status: hoe hoger de status, hoe lager de kans (sociale selectie; downward mobility (van hoge naar lage klasse))
- Communicatiestijl: eerdere terugval als:
- Familieleden veel kritiek hebben op patiënt
- Familieleden vijandig tegenover de patiënt staan
- Familieleden emotioneel overbetrokken zijn bij de patiënt
Therapie van schizofrenie kan biologisch met farmacotherapie, rTMS (repetitive transcranial magnetic stimulation) en electroconvulsive therapie (shock therapie).
Therapie van schizofrenie kan ook psychosociaal met sociale vaardigheidstraining, cognitieve gedragstherapie, familietherapie en coaches.
