Zwangerschap van meerlingen
Meerlingenzwangerschappen geven een verhoogde kans op het krijgen van maternale complicaties en een verhoogde kans op morbiditeit en mortaliteit van de kinderen. De risico´s bestaan uit het krijgen van pre-eclampsie, vroeggeboorte en groeivertraging. Er is ongeveer 1 procent kans op het krijgen van een spontane tweelingzwangerschap. Bij ovariële stimulatie en IVF en ICSI is de kans op een tweelingzwangerschap verhoogd.
Maternale complicaties
De maternale complicaties van meerlingenzwangerschap bestaat onder andere een verhoogde kans op hyperemesis, anemie, zwangerschapsdiabetes en hypertensie, bloeding door abruptio placentae en verhoogde complicaties die gepaard gaan met een verhoogde kans op een sectio caesarea.
Diagnostiek
Een meerlingzwangerschap wordt bijna altijd echoscopisch gesteld. Een echo geeft een hoge sensitiviteit en specificiteit. Er wordt geadviseerd om ook voor tweelingzwangerschappen een eerste trimester combinatietest te verrichten. Tevens worden de nekplooimeting (NT) gemeten en wordt er gekeken naar de maternale leeftijd.
Bij dichoriale tweelingzwangerschappen is vroeggeboorte het meest voorkomende probleem. Het bleek dat profylactische cervixcerclage, klinische opname en/of weeënremming niet effectief zijn bij vroeggeboorte. Door uteroplacentaire factoren is de kans op groeirestrictie vergroot in vergelijking met eenlingzwangerschappen. Vanaf zevenendertig weken neemt de kans op intra-uteriene vruchtdood bij tweelingen sterk toe. Toch zijn er onvoldoende data om een beleid van routinematig inleiden bij zevenendertig weken in geval van ongecompliceerde dichoriale tweelingzwangerschap uit te voeren. Bij de bevalling van een tweeling moet er een gynaecoloog aanwezig zijn.
Actief beleid
Uit verschillende onderzoeken blijkt dat een actief beleid de voorkeur verdient. Een interval van langer dan dertig minuten wordt geassocieerd met een grotere kans op asfyxie. Daarom wordt het aangeraden om een actief beleid te voeren na de geboorte van het kind. Dit actieve beleid bestaat uit een oxytocine-infuus en te overwegen om bij een kind in stuitligging een stuitextractie te verrichten. Rond de zesendertig à zevenendertig weken is het zinvol om een ongecompliceerde monochoriale diamniotische tweelingzwangerschap routinematig in te leiden.
