Is cognitieve gedragstherapie (CBT) effectief op de lange termijn voor personen met een ultrahoog risico op het ontwikkelen van een psychose?
Introductie (achtergrondgegevens en eerder onderzoek)
Sinds de vaststelling van ultrahoge risico (UHR) criteria voor het ontwikkelen van een psychose is het doel om het begin van een eerste episode van een psychose bij UHR-individuen te voorkomen. Een recente meta-analyse van de effecten van cognitieve gedragstherapie (CGT) voor jongeren bij UHR liet veelbelovende resultaten zien: CGT werd geassocieerd met een algehele risicovermindering van 54% op de ontwikkeling van een psychose bij een follow-up van twaalf maanden en 37% bij een follow-up van 24 tot 48 maanden. Naast de klinische voordelen van preventie van psychose, kunnen de zorgkosten ook worden verlaagd door preventie van psychose.
In lijn met recent onderzoek, toonden de auteurs in een eerder onderzoek al aan dat CGT effectief is bij het voorkomen van een psychose in de Nederlandse Early Detection Intervention and Evaluation (EDIE-NL). Cognitieve gedragstherapie voor ultrahoog risico (CBTuhr genoemd) halveerde de incidentie van psychose gedurende een periode van 18 maanden.
De huidige studie onderzoekt de 4-jarige follow-up gegevens van dezelfde studie. Het doel van deze studie is om te evalueren: (1) of CBTuhr preventieve effecten heeft gehad voor de eerste episode van een psychose, (2) of CBTuhr remissie bevordert vanuit de UHR-status, en (3) de klinische en functionele status van degenen die zich wel tot psychose hebben bekeerd.
Methode
De Nederlandse vroegtijdige detectie- en interventie-evaluatiestudie was een gerandomiseerde gecontroleerde studie van 196 UHR-patiënten waarin CBTuhr werd vergeleken met de behandeling zoals gebruikelijk (TAU) voor comorbide stoornissen met alleen TAU. Van de oorspronkelijke 196 patiënten stemden 113 in met een follow-up van 4 jaar (57,7%; CBTuhr = 56 versus TAU = 57). Tijdens de studieperiode werden de incidentie van psychose, remissie van de UHR-status en de effecten van de overgang naar psychose geëvalueerd.
Resultaten
Het aantal deelnemers in de CBTuhr-groep dat een psychose ontwikkelde, steeg van 10 na 18 maanden naar 12 na 4 jaar follow-up, terwijl het niet veranderde in de TAU-groep (n = 22). Dit verschil is significant. De odds ratio van CBTuhr vergeleken met TAU was 0,44 (95% BI: 0,24-0,82). Bovendien, aanzienlijk meer patiënten remitteerden van hun UHR-status in de CBTuhr-groep (76,3%) vergeleken met de TAU-groep (58,7%) [t (120) = 2,08, P = 0,04]. Belangrijk is dat de overgang naar psychose geassocieerd werd met ernstiger psychopathologie en sociaal functioneren na 4 jaar follow-up.
Conclusie
De algemene conclusie is dat CBTuhr ook na 4 jaar nog steeds effectief is om een eerste episode van psychose te voorkomen bij personen met een ultrahoog risico op het ontwikkelen van een psychose.
