Wat is een duale diagnose?
In een duale diagnose is er sprake van een middelenverslaving tezamen met een andere psychiatrische stoornis. Deze combinatie is erg gevaarlijk voor de gezondheid en de samenleving en er bestaan grote kansen op herhaling van middelengerelateerd gedrag en destructieve levensstijlen. De stoornissen bij een duale diagnose kunnen een gezamenlijke oorzaak hebben, of elkaar versterken. De causale relaties zijn niet altijd duidelijk te achterhalen. De symptomen die vaak worden gezien bij de psychische stoornissen verwijzen naar problemen bij het executief functioneren.
Wat is executief functioneren?
Executief functioneren verwijst naar alle cognitieve processen die gedrag reguleren op zo’n manier dat het efficiënt en doelgericht kan zijn. Er worden drie aspecten van executief functioneren onderscheiden:
Shifting. Dit verwijst naar de verschuiving van aandacht tussen taken en mentale sets. Het betreft mentale flexibiliteit om herhaaldelijk van irrelevant of ongewenst gedrag af te zien en over te gaan op meer relevant en geschikt gedrag.
Updating. Dit verwijst naar het proces van het actief manipuleren en monitoren van relevante informatie in het werkgeheugen, zodat oude informatie wordt vergeten of geactualiseerd en nieuwe informatie kan worden opgeslagen.
Inhibition. Dit verwijst naar de vaardigheid om van dominant en automatisch gedrag af te zien wanneer dat nodig is.
Deze drie aspecten zijn alle drie nodig bij het maken van alledaagse beslissingen. Tekorten in een van deze aspecten kunnen leiden tot het ontstaan van mentale stoornissen. Bijvoorbeeld door niet doelgericht te kunnen functioneren doordat iemand niet van bepaalde wensen kan afzien of doordat impulsen niet kunnen worden weerstaan.
Welke belangrijke resultaten zijn er gevonden met betrekking tot middelenverslavingen en comorbiditeit met andere psychische stoornissen?
Vergeleken met een gezonde controlegroep blijken patiënten met een duale diagnose vooral beperkingen te hebben met shifting en inhibitory control. Het is waarschijnlijk dat het voor deze mensen lastig is om een realistisch beeld te hebben van alledaagse situaties en daarin flexibel te participeren en tijdig inhibitie te vertonen, omdat zowel middelengebruik als de symptomen van de andere stoornissen verschillende hersenprocessen uit balans brengen.
Vergeleken met patiënten met schizofrenie en zonder een middelenverslaving blijken patiënten met schizofrenie en een middelenverslaving minder inhibitory control te hebben. Dit zou kunnen komen doordat de patiënten die zowel lijden aan schizofrenie als een middelenverslaving door dat middelengebruik minder in staat zijn om hun impulsen te weerstaan.
Patiënten met een middelenverslaving en schizofrenie functioneren beter op het shifting aspect van executief functioneren dan de patiënten met schizofrenie en zonder een middelenverslaving. Dit zou kunnen worden verklaard doordat de patiënten met schizofrenie en een middelenverslaving impulsiever zijn en meer op zoek zijn naar nieuwe prikkels dan de patiënten die alleen lijden aan schizofrenie maar daarbij niet tevens lijden aan een middelenverslaving.
