Waar gaat dit artikel over?
Dit artikel is een systematische review en meta-analyse. Deze studie is ontwikkeld om het bewijsmateriaal uit gepubliceerde literatuur bij te werken en een overzicht te geven van de effectiviteit van cognitieve gedragstherapie (CBT) bij de behandeling van somatoforme stoornissen en medisch onverklaarde lichamelijke symptomen (in het Engels afgekort tot MUPS).
Hoe is de literatuur onderzocht?
Het protocol van deze review is online beschikbaar op PROSPERO. De zoekstrategie is uitgevoerd in verschillende databases: Cochrane, MEDLINE (via PubMed), EMBASE, PsycINFO, Scopus, en verschillende Chinese databases (waaronder ook een database met grijze literatuur). Naast het zoeken is deze databases is de sneeuwbalmethode toegepast. Dit betekent dat er is gekeken in de referentielijst van originele zoekresultaten naar mogelijk relevante (gemiste) artikelen.
De volgende inclusiecriteria werden gehanteerd:
- De diagnose betreft een somatische stoornis.
- De participanten zijn minimaal 18 jaar oud.
- De interventie was CGT versus gebruikelijke zorg, verbeterde zorg of wachtlijst.
- Het design van de studie omvatte een gerandomiseerd, gecontroleerd design, ofwel RCT (randomized controlled trial).
- Wanneer meerdere studies met dezelfde steekproef zijn gepubliceerd, dan werd alleen de meest recente opgenomen in de analyse.
De volgende informatie werd uit ieder artikel afgeleid: eerste auteur, publicatiejaar, publicatiestatus, studieontwerp, inclusie- / exclusiecriteria, onderzoekspopulatie, deelnemersafhankelijkheid, behandelvorm, frequentie en duur van de behandeling, naleving van CBT, controlecondities, uitkomst van de interventie, meet en informatie perioden voor beoordeling van het risico van bias. Het primaire resultaat was de ernst van somatische symptomen; secundaire uitkomsten waren ernst van angstsymptomen, ernst van depressieve symptomen, sociaal functioneren, fysiek functioneren, doktersbezoeken en therapietrouw, evenals vervolgbezoeken.
Aanvankelijk werden 1107 artikelen geïdentificeerd. Daarnaast werden er nog vier artikelen via andere bronnen geïdentificeerd. Na een uitgebreid selectieproces bleven er uiteindelijk vijftien RCT's over (met in totaal N = 1671 patiënten), die zijn opgenomen in de kwantitatieve analyse (de meta-analyse). Van deze vijftien RCT's, werden er drie uitgevoerd in de VS, drie in Nederland, twee in Denemarken, twee in Zweden, twee in Sri Lanka, en één in Duitsland, Schotland, en Spanje. Iedere studie bestond uit 31 tot 172 participanten, met een gemiddelde leeftijd van 41.77 jaar (de range varieerde van 34 tot 49.2 jaar).
Is CBT effectief bij de behandeling van somatoforme stoornissen?
De hoofd analyse toonde aan dat CBT somatische symptomen kan verlichten: gemiddeld verschil / odds ratio = −1.31 (95% BI: −2.23 tot −0.39, p = 0.005); angstsymptomen: -1,89 (95% BI: −2,91 tot −0,86; p <0,001); depressieve symptomen: −1,93 (95% BI: −3,56 tot −0,31; p = 0,020); verbeteren fysiek functioneren: 4,19 (95% BI: 1,90 tot 6,49; p <0,001). De effectiviteit van CGT op het verlichten van somatische symptomen, angst en depressieve symptomen hield ook op de lange termijn aan, zoals blijkt uit de resultaten van de follow-up metingen.
CGT is mogelijk niet effectief bij het verminderen van het aantal doktersbezoeken: −1.23 (95% BI: −2,97 tot 0,51; p = 0,166); en verbetering van het sociaal functioneren: 3,27 (95% BI: −0,08 tot 6,63; p = 0,056).
De resultaten van subgroepanalyse gaven aan dat CGT vooral nuttig was wanneer de duur van de sessie meer dan 50 minuten bedroeg om de ernst van de somatische symptomen te verminderen van vóór tot na de behandelingstijd, wanneer de behandeling (1) in groepsverband werd toegepast, (2) met affect en (3) er goede interpersoonlijke strategieën werden ontwikkeld tijdens de behandeling. Langere duur en frequentie, zoals meer dan tien sessies en twaalf weken behandelingen, hadden een significant effect op de vermindering van de comorbide symptomen, waaronder depressie en angst, maar ze kunnen de lage mate van therapietrouw van op CBT gebaseerde behandelingen ondersteunen (met andere woorden: patiënten zijn eerder geneigd om vroegtijdig te stoppen met de behandeling).
De algemene conclusie is dat CBT effectief is voor de behandeling van somatoforme stoornissen en MUPS door het verminderen van fysieke symptomen en psychologische behoeften en gebreken.
