Antipsychotica zijn de eerste behandeling voor schizofrenie, omdat ze de symptomen kunnen verminderen. Ze verminderen ook het aantal sterftegevallen, omdat ze suïcidaliteit en terugval tegengaan. Maar vele patiënten weigeren hun medicijnen in te nemen of zijn nalatig. Dit komt veelal door de bijwerkingen. Bovendien laat onderzoek zien dat het positieve effect van antipsychotica voor schizofrenie overschat wordt. Er is wel een effect, maar veel kleiner dan verwacht. Cognitieve therapie blijkt in combinatie met antipsychotica effectief te zijn. Morrison et al. (2014) hebben gekeken of cognitieve therapie effectief is in het verminderen van psychische symptomen bij schizofrenie patiënten die geen medicatie gebruiken.
Zij hebben een single-blind gerandomiseerde controle onderzoek gedaan in twee klinieken in Engeland. Dit onderzoek werd gedaan onder 74 Participanten in de leeftijd 16 tot 65 jaar met een diagnose schizofrenie, schizo-affectieve stoornis of waanstoornis, of wanneer zij voldeden aan de criteria voor een vroege interventie voor psychoses. Deze participanten hadden zelf gekozen om geen medicijnen te gebruiken voor hun psychotische symptomen. Door een computersysteem werden zij tot één van de volgende groepen verdeeld: 1. Cognitieve therapie in combinatie met de normale behandeling of 2. alleen de normale behandeling. Daarna werd er gekeken naar de totale score op de positive and negative syndrome scale (PANSS). Deze werd afgenomen aan het begin (baseline) en op maand 3, 6, 9, 12, 15 en 18.
De gemiddelde PANSS score waren significant lager in de cognitieve behandeling groep (1) dan in de groep die een normale behandeling kreeg (2). De resultaten laten zien dat cognitieve therapie psychiatrische symptomen significant vermindert en sociale en persoonlijke functioneren verbetert. Ook heeft het een positief effect op sommige dimensies van waanideeën (cognitief) en het horen van geluiden. Maar therapie blijkt geen invloed te hebben op de stress die gepaard gaat met waanideeën, op depressie, sociale angst en zelf beoordeelde herstel.
Er waren in totaal acht ernstige gevallen (bijwerkingen). Zo was er in de cognitieve therapie groep na cognitieve therapie één persoon die een overdosis innam en één persoon die een risico vormde voor anderen. In de groep die de normale behandeling kreeg waren er zes ernstige gevallen (twee sterftegevallen, drie verplichte opnames en één overdosis).
Morrison et al. stellen dat er op basis van de uitkomsten van dit onderzoek gesteld kan worden dat cognitieve therapie psychische klachten significant verminderd en een veilig en acceptabele alternatieve keuze kan zijn voor patiënten met schizofrenie die geen antipsychotica willen gebruiken. Dit onderzoek was echter slechts een pilot studie. Er is dus meer onderzoek nodig.
